The Nurse Practitioner (NP) has a specialized skill and knowledge related to the setting in which they practice and perform physical exams, collects and documents data, conducts approved diagnostic and therapeutic procedures, orders and schedules laboratory studies and professional consultations, prescribes appropriate interventions and medications, and provides direct patient care services, according to established evidence-based clinical practice standards. The NP contributes to excellence in patient care, research, teaching, consulting, and provides leadership to the organization and advance practice profession. Duties may include but not limited to obtaining patient health histories and complete physical examinations, documenting findings noting pertinent normal and abnormal findings, ordering and interpreting appropriate diagnostic and laboratory tests, and performing procedures as needed within specialty area. The NP also helps facilitate safe and efficient operational flow of the patient through their continuum of care. The NP will function as a part of inter-professional teams by communicating, planning, and implementing care directly with other healthcare professionals, including but not limited to physicians, pharmacists, office support staff, and other advance practice providers (APPs). The Nurse Practitioner (NP) provides advanced clinical care for patients across the heart transplant and mechanical circulatory support (MCS/VAD) continuum, including pre-transplant evaluation, waitlist management, post-transplant follow-up, and durable VAD therapy. The NP works in collaboration with the multidisciplinary transplant and MCS teams to deliver safe, high-quality patient care in both inpatient and outpatient settings. Responsibilities include performing comprehensive clinical assessments, participating in selection committee activities, supporting cross-coverage across transplant and MCS services, contributing to staff and patient education, and maintaining accurate documentation and data to support regulatory and reporting requirements (CMS, TJC, INTERMACS, UNOS/UNet). Job Summary: The ICD Nurse Specialist role at Kaiser Permanente Southern California Region is an advanced practice nursing role with the critical element being expert clinical practice. Essential Responsibilities: To ensure that nursing care needs of the cardiac device patient population are met, provides 24 hour clinical leadership by developing standards of practice, policies and procedures for cardiac device patient population that are consistent with professional practice standards. Implementing methods and processes in specialty area to support change in nursing practice, care programs and clinical innovation. Achieving improved patient/family outcomes. Independently assess cardiac device transmissions, including electrograms, from patients using knowledge and experience in device programming. Functions independently in the assessment and interpretation of cardiac device data from patients in outpatient clinic, inpatient hospital, and by remote interrogation. Provides medical and/or technical programming recommendations and advice. Performs interrogations and reprogramming as needed, on all types of implantable cardiac devices. Monday through Friday 8 to 5pm, no nights, weekends, or holidays! The Nurse Practitioner provides primary care to enrolled participants by evaluating participant physical complaints and providing appropriate treatment. The Nurse Practitioner functions beyond as an advanced practice nurse and as an integral member of the interdisciplinary team. Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential to performing the job duties. Evaluates participants and documents information in participant’s chart. Performs physical examination and orders and interprets diagnostic tests. Performs blood draws, EKG’s, dressing changes and any other nursing interventions as needed. Formulates an assessment of participant’s condition and provides a treatment plan, including writing prescription(s) and providing ongoing follow-up of condition. Provides alternative methods of therapy to participants as appropriate. Educates staff, participants and caregivers regarding healthcare issues and treatments. Manages acute and long-term medical needs of participants in collaboration with other primary care providers and oversees the care eds of participants in nursing homes, homes, Assisted Living Facilities and/or group clinic settings. Provides emotional and spiritual support to participants. Provides management of medical emergencies as needs dictate. Ensures appropriate and timely documentation of medical care in the participant’s medical record, in compliance with InnovAge Greater PACE documentation policies. Identifies documents and corrects problems in medical records.
