Job Overview The primary function of the Home Health Infusion Nurse is to provide exceptional in-home IV infusion therapy, nursing care, and response management. In addition, the Home Health Infusion Nurse will administer and monitor patients in the home during treatment and provide the necessary education on treatment regimen, condition management and treatment self-administration when appropriate. Partner with the patient, family and care team in developing and implementing plan of care with specific goals under the direction of the physician. Initiate and coordinate care conferences with the participation of all members of the care team as needed. Provide education and support to patients and caregivers that enable them to safely and effectively self-administer medications and solutions. This includes storage, aseptic technique, equipment use, side effects and potential complications. Coordinate the discharge of patients when goals are met with the physician and other disciplines to achieve an orderly transition for the patient/family. Train patients in the self-administration of treatments and how to quickly recognize and respond to symptoms of adverse physical reactions to treatments. About This Unit: Looking for an opportunity to gain experience on a cardiovascular unit? This is a 33 bed Heart Care unit with rapid turnaround for patients that stay on average for 2.5 days. Our typical patient population consists of patients with observable chest pain, ruling out anything cardiac related, stroke patients, and alcohol withdrawal. We prepare and recover our patients for stress tests, echocardiograms, and cardiac procedures including diagnostic angiograms/stent placement, ablations, pacemakers and ICD insertions, and open heart surgery. Our unit cares for many patients with heart failure and cardiac arrythmias needing IV drips. A member of the interdependent team led by a designated RN, is accountable to perform patient care duties and activities for a group of patients. Performs such care as vital sign checks, bladder scanning, blood sugar testing, documentation of cares provided. Reports patient care data and progress of outcomes to designated RN.
