As part of our interdisciplinary care team, the ideal candidate will be a compassionate, detail-oriented professional who is experienced in conducting in-person assessments, developing individualized care plans, and working closely with residents, families, and healthcare providers to promote resident well-being and regulatory compliance. Duties: Perform thorough in-person clinical evaluations for existing residents according to UAS standards, agency policies, and state laws. Create individualized service plans and determine eligibility for ongoing participation through detailed UAS assessments. Evaluate potential new residents by considering clinical, social, and environmental factors to establish appropriate levels of care ensuring safety and well-being.
