Provides general and/or specialized nursing care to patients with Acute and Chronic Kidney Disease or Apheresis needs in the ambulatory and inpatient setting. Provides nursing care to patients and families in the Dialysis and Infusion Center in accordance with the ANA standards of practice of a professional nurse, Connecticut Children’s policies and procedures, SCOPE dialysis protocols and State and federal regulations. Utilizes a primary nurse model. Reports to Nurse Manager of the Dialysis Center. CLINICAL PRACTICE - Demonstrates clinical expertise and uses the nursing process to provide safe, appropriate patient care. Prioritizes, plans, provides holistic family-centered care to patients within area of service throughout the continuum of care utilizing time and resources effectively and efficiently. Works with the Nurse Manager of Dialysis Center to assure SCOPE and ANA standards are upheld. Procedural proficiencies in multiple modalities will be required for acute and chronic needs. Must be able to work within a multidisciplinary team environment. POPULATION SPECIFIC - The Dialysis/Nephrology nurse has a knowledge of pediatrics, pathophysiology of ESRD and apheresis therapies. Assesses the patient response to hemodialysis and peritoneal dialysis therapy. Communicates problems and concerns to the physician and engages appropriate specialty team members as needed. Demonstrates cultural sensitivity in all interactions with patients/families. UNIT LEADERSHIP - As assigned, contributes to the leadership of the unit by functioning as the Charge Nurse and/or by participating in the professional development of others through preceptor/mentor activities. Supports the unit/department’s work productivity and resource management activities. Actively participates in quality/safety initiatives. DOCUMENTATION/COMMUNICATION – Timely, accurate and succinct communication required for optimal care. Appropriate use of paging, texting, and staff messaging. Clearly documents all pertinent aspects of care, including but not limited to : daily huddles, treatment plans, weekly and monthly summary of patient status; unexpected events of changes in patient condition; medications; and monitoring of vital signs in a timely manner and according to policies and standards; necessary information is effectively communicated to all members of the health care team. PATIENT/FAMILY PARTNERSHIP & EDUCATION - Collaborates with child and family in evaluating outcomes. Provides appropriate, age/developmentally appropriate education to patients and families regarding all aspects of care according to policies and standards; supports the organization’s efforts to patient/family education. Partners with child and family in evaluating outcomes. Monitors, trends and evaluates child/family responses to treatment throughout scope of service. Provides appropriate education to patient/family regarding all aspects of care, including telephone advice, based upon established protocols. Serves as advocate for parents and families. CONNECTICUT CHILDREN’S STANDARDS - Demonstrates support for the mission, values and goals of the organization through behaviors that are consistent with organizational standards. Supervisory Responsibilities: Charge Nurse duties as outlined in the Dialysis Center. Education and/or Experience Required: A minimum of ASN/ADN required upon hire, BSN preferred. If ASN/ADN, must acquire BSN within 3 years of hire Education and/or Experience Preferred: Experience Preferred: Dialysis Experience preferred. Apheresis Experience preferred. Pediatric experience preferred. Must acquire (within 6 months) a thorough understanding of water purification standards to assure compliance with the AAMI guidelines. Follows the CT Nurse Practice Act and follows McLean Nursing Philosophy of Excellence, Leadership, Integration, Learning and Discovery, while administering and directing ancillary personnel in the performance of nursing tasks. Takes responsibility for the delivery of care on the unit and engages in proper hand-off to the next shift. JOB FUNCTIONS: (This list may not include all job duties assigned.) 1. Arrives for duty on time and ready to assume responsibility for the unit when assigned. Organizes resident/patient care according to accepted routines and prioritizes activities, and manages work day so as to complete duties within eight hour shift. 2. Responsible and will be held accountable for ongoing assessments of the residents’ nursing care needs. Immediately reports changes in condition of residents with accurate assessments and interventions to supervision, APRN or physician. Transcribes orders from W10. Notifies physician in timely manner of consultant/or post physician visit. Assists the MDS Coordinator in the development of the plan of care for each resident/patient. Provides nursing care to residents in accordance with standard nursing practice. This care will include the administration of medications and patient treatments as prescribed by the physician. Documents care and resident response to medications and nursing interventions. Assists in the provision of patient/family teaching and education. Documents patient understanding in the medical record. This includes both formal and informal teaching sessions. Examples of teaching include prescribed medications, activities of daily living, treatments and procedures. Takes and assesses vital signs as indicated by the plan of care or change in patient condition. Responsible to wear care unit pager at all times and respond when signaled, except during lunch period. Responsible to read and follow ECS messaging . Knows and is able to carry out approved emergency procedures. Responsible for and prepares As & Is and follows-up to completion. Monitors IV therapy, documents, and discontinues IV therapy. Follows Standard Precautions and ensures procedures are followed by ancillary staff. Participates in and/or provides information for resident planning conferences. Communicates resident information to other members of the healthcare team and to resident’s family. Maintains a safe environment for residents, staff, and visitors and reports any concerns to supervisor. Assigns resident assignments for ancillary staff. Reviews and monitors patient care performed by ancillary personnel. Collects and documents quality care indicators. Provides feedback to supervision. 18. Promotes the mission and values of McLean, and exhibits professional manner at all times in both speech and body language. Supports department and unit initiatives. 19. Manages time appropriately, completes duties within 8-hour shift and/or requests supervisory assistance when necessary to ensure compliance, prior to end of shift. 20. Participates in continuous learning opportunities both professionally and personally. Notifies supervisor or nurse educator on learning needs to perform nursing care. Abides by McLean Nursing Philosophy, policies and procedures of the Nursing department. Performs other activities as assigned. WORKING CONDITIONS: Typical Physical Demands : Requires frequent rolling greater than 80 pounds, often combined with bending and turning. Must be able to push/pull a loaded medication cart over carpeted floor for extended distances for significant periods of time. Requires the ability to respond, direct and intervene during stat calls throughout building in a timely manner. Requires ability to walk up and down stairs. Requires corrected vision and hearing within normal limits. Requires use of office equipment such as computers, telephones, fax machines and copy machines. Requires use of medical equipment in the clinical setting. Requires eye/hand coordination and manual dexterity. Requires frequent prolonged standing, walking, and the ability to perform physical labor for an eight-hour day. Typical Working Conditions : Work is primarily performed in a long-term care facility. Contact with staff, residents, families, physicians, police, clergy, ambulance crews, pharmacists, vendors, volunteers, and students. May be required to work with uncooperative and combative residents. EMPLOYMENT REQUIREMENTS: Knowledge, Skills, Abilities : Knowledge of nursing theory and practice. Knowledge and competency in the nursing process, quality assurance, infection control, hospice philosophy, restorative, rehabilitative, and gerontological nursing practice and theory. Able to interpret written instructions, communicate and document effectively. Ability to assess, plan, implement, evaluate, and document care and instructions. Ability to prioritize appropriately and make sound nursing judgments. Demonstrates personal integrity and empathy and fosters such in others. Education: Graduate of an accredited Registered Nurse program. Certification/Licensure: Current Connecticut State License in good standing as a Registered Nurse. Experience: Minimum of one-year experience in LTC, working with geriatric population.

Qualifications EDUCATION Graduation from an accredited school of nursing. BSN preferred.
