The Nurse Navigator is a consistent care coordinator throughout the continuum of care and assess the physical, psychological, and social needs of the patient enhancing patient outcomes, increasing satisfaction, and reducing cost of care. The Nurse Navigator is responsible for community education and screening as well as monitoring community outreach and promoting survivorship. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. Current and valid state RN License. 2. Current BLS certificate upon hire and maintain current. 3. Bachelor of Science in Nursing (BSN) preferred. 4. Minimum of two (2) years of nursing experience.
