Description The Medical Assistant II provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Mentors and assists less experienced staff members. Responsibilities Fulfills patient care responsibilities as assigned which may include: performing venipuncture and/or EKGs, checking schedules and organizing patient flow; accompanying patients to exam/procedure room, assisting patients as needed with walking, transferring and dressing, as well as collecting and processing specimens and point of care testing, preparing for exam, etc. Assists physicians with various procedures, takes vital signs, relays instructions to patients and or families; answers calls and provides pertinent information. Establishes a plan of care with input from appropriate multidisciplinary health team members based on goals mutually acceptable to the patient and family. Documents content and process of all services provided. Monitors the effect of the care plans on the patient health status and informs physician and other personnel of changes in the clients’ needs. Appraises care provided, revises goals and interventions to patients’ care as needed. Educates the patient and family regarding the disease process, self-care techniques and prevention strategies. Initiates and maintains written and verbal communications to assure optimal patient care. Provides supervision of workers providing health and personal care services through visits to patients’ home. Maintains updated Home Health Aide (HHA) plan of care and reviews same with HHA and patient/family. Communicates with HHA on a regular basis regarding interdisciplinary plan of care, patients’ goals and progress. Documents initial and bi-weekly supervision of HHA services. Plans, facilitates and documents patients’ discharge from the program. Manages caseload as assigned in Manhattan.
