We provide quality hospice care to patients in skilled nursing facilities, assisted living facilities and in private homes. Job Responsibilities Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse. Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient. Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails. Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies. Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry. Feeds and assists with meals. Obtains temperature, pulse and respiration as supervised by the Hospice Nurse. Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information , including the date and time of the report and the person receiving the information. Ideal candidates will have: HHA License in the state of NJ Understands the hospice philosophy Minimum of one year experience working in hospice Dependable transportation

Quality and Safety Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. Implements process improvement strategies based on institutional, evidence-based ensure and procedures. Engages in formal and informal peer and institutional review processes. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies. Contributes to quality and safety practices and required compliance measures. Implements principles of high reliability to identify and avoid high risk behaviors to provide a safe environment for patients. Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol. Utilizes institutional information technology by documenting nursing practice to support quality and performance improvement initiatives. Operations Performs charge nurse duties as assigned. Coordinates with the Clinical Coordinator/Clinical Nurse Manager/Assistant Nurse Manager/Nursing Administrator to ensure supplies are appropriately utilized and inventory is sufficient for patient care. Escalates material or staffing resource deficits to charge nurse, Clinical Nurse Manager/Assistant Nurse Manager or Nursing Administrator. Demonstrates patient focused and cost-effective approaches to patient care in terms of equipment, staff, supplies and all other resources. Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care. Facilitates patient throughput on a shift basis. Professional Development Contributes to the environment of care to support clinical colleagues, patients and their families and members of the healthcare team. Advances clinical competence in nursing practice to progress from novice to expert. Projects a professional image to colleagues and communicates with styles and methods that demonstrate caring, respect, compassion and empathy. (BSN) Associate’s Degree in Nursing (ADN) RN who is matriculated in a BSN Program with a graduation date within the year. Any Associate’s Degree in Nursing (ADN) RN with Nursing experience must be approved by the CNO. Job Description Position Summary: The Medical Assistant (MA) is responsible for assisting physician, PA or NP with clerical duties, patient flow, patient care and procedures. Essential Job Functions: Prepares the examination room. Escorts patient into the room. Cleans examination room when exam is complete. Maintains efficient patient flow. Ensures proper preparation for patient visit. Prepares pertinent information needed for patient visit. Communicates & provides care consistent with age, cultural, spiritual and developmental needs of the patient. Answers the telephone in an appropriate manner. Directs calls and/or takes accurate messages. Schedules appointments accurately and appropriately according to departmental guidelines. Consistently and accurately stocks examination rooms with all necessary supplies. Assists in ordering and maintaining of all supplies as assigned. Reconciles encounters daily and accurately, according to policy guidelines. Effectively communicates problems, concerns or issues to the Supervisor and/or Manager appropriately and promptly. Understands and performs within scope of practice. Obtains vital signs: BP, temp, pulse, height & weight accurately and completely as required. Documents appropriately in EHR. Reviews and updates medication list and accurately documents known allergies in the Electronic Health Record (EHR). Demonstrates an understanding of prescription control and prescription refill procedures. Accurately documents allergies in EHR. Appropriately enters orders and schedules appointments in the Practice Management System (PMS) and EHR. Provides referrals and/or consult orders appropriately and in a timely manner in the PMS and EHR. Appropriately adheres to universal safety precautions when administering medications and disposal of medical waste. Maintains proficiency with department specific equipment, troubleshoots and reports malfunctions promptly Facilitates transition of care to UCC/Hospital Demonstrates flexibility with various work schedules. All other duties as assigned Recognizes and performs duties which need to be performed although not directly assigned. Provide scribing services. Education, Certification, Computer & Training Requirements: High School Diploma or equivalent, required. Graduated from an accredited medical assistant or phlebotomy program, required. 0-1 year related work experience, required. 2-4 years related work experience, preferred. Basic Life Support (BLS) or within 30 days of hire, required Ability to communicate in English, both orally and in writing, required Ability to organize and perform multiple tasks in a timely manner, required Basic proficiency in computer use, required Travel : Ability to commute to satellite offices as needed, required Pay Range: $22.02 - $27.07 Hourly The provided compensation range is based on industry standards and salary determinations will be made based on numerous factors including but not limited to years of experience and location of position. PATIENT CARE Patient care responsibilities are delivered with a knowledge of patient growth and development and are appropriate to the ages of the patients served. Observes and reports any changes in the residents’ physical, mental, emotional, Behavioral, and social well-being. Takes and records TPR and reports deviations from normal to Primary or Charge Nurse. Obtains specimens as assigned (e.g., urine, stool, sputum). Weighs patient and records weight. Assists patient with activities of daily living including skin care (e.g., bathes, dresses, helps patients ambulate). Records oral intake and urinary output. Performs assigned treatments (e.g., R.O.M., Dayroom Monitoring, Courtesy Cart). Communicates effectively with Charge Nurse/Primary Nurse regarding patients’ needs and requests. Orients patient to environment and develops rapport with patients, family and significant others. B. PATIENT CARE RELATED DUTIES Keeps patient unit and non-patient unit clean (e.g., wipes tables, neatens storage rooms, pantries and other areas). Prepares bedside units for new admissions (e.g., stocks with bedpan, tissues, water, cups). Apply principles of infection control and safety in the work environment. Reports significant unit problems to charge nurse. Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards. Promotes adherence to the Patient's Bill of Rights. Customer Service: respect, flexibility, knowledge, confidence, professionalism, pleasant attitude, patience and helpfulness. Must pass Nursing Attendant test administered by BNBMC Speaks, reads and writes English to the extent required by the position. Education: Successful completion of an approved Nurse’s Aide training course required. High school graduation or equivalent preferred. Salary commensurate with experience within posted range.
