Coordinates patient care by working collaboratively with the interdisciplinary team, patients and families. Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patient population served. Promotes the profession of nursing and professional practice through collegial support and interactions. Reports to the Clinical Nurse Manager. COMPETENCIES: Must be able to communicate effectively in English, both verbally and in writing and possess basic computer skills. ESSENTIAL DUTIES and RESPONSIBILITIES: Job-Specific Competency 1. Performs population-specific comprehensive assessments and re-assessments of the patient’s condition and care needs including the psychosocial, physical, educational and cultural aspects. 2. Utilizing assessment data, collaborates with surgical team to define, implement and evaluate an individualized plan of care for each surgical patient. 3. Follows the five (5) medication rights and reduces the potential for medication errors. Demonstrates knowledge of medications and their correct administration based on age of the patient and their clinical condition. Follows Surgical Services policy for medication safety. 4. Performs all aspects of patient care in an environment that optimizes patient safety and promotes positive patient outcomes; follows ECHN policy for Patient Identification, Surgical & Invasive Procedures Site Identification, and Time-Out. 5. Documents the nursing process and responses to care in a timely manner utilizing the appropriate tools. 6. Demonstrates an ability to be flexible, organized and function under stressful situations. Utilizes critical thinking skills and sound judgment in priority setting and delegation. 7. Demonstrates autonomous practice, consistent with evidence-based standards. Pursues personal and professional growth and development. Serves as a professional role model and mentor. 8. Demonstrates knowledge of the principles of growth and development over the lifespan and the skills necessary to provide age-appropriate care to patients served. Able to interpret data about the patient status in order to identify each patient’s age-specific needs and provide care needed by the patient group. 9. Provides an accurate and pertinent report of the patient’s condition for safe transfer of care. Prepares room in a timely manner with all anticipated instruments, sutures, supplies, and equipment for each surgical procedure. Demonstrates knowledge of and assures maintenance of aseptic technique throughout the surgical procedure; identifies and communicates, to surgical team, any breaks in technique and assists with corrective actions. Effectively communicates with the surgeon in accounting for all sponges, needles, and instruments; reports discrepancies immediately to surgeon and Team Leader; appropriately documents all counts. Accurately records, handles, labels and processes specimens. Demonstrates ability to effectively function in the scrub role. Qualifications EDUCATION/CERTIFICATION: Must be a graduate of an accredited school of Nursing and have and maintain current a Registered Nurse license in the State of Connecticut. Must have certification in Cardio Pulmonary Resuscitation (CPR) upon hire. EXPERIENCE: Previous surgical or labor and delivery, specifically assisting with C-sections, experience required. Maintain current resident care plans on all residents under his/her care. Follow physician’s orders. Renders and/or assists with complete nursing care, treatments, dressings, and other physician orders. Transcribe physician’s orders promptly and accurately. Review patient records daily to assure accuracy and completeness. Document comprehensive and complete nursing notes. Maintain accurate awareness of residents on alert list/conduct and document RN assessments as needed. Document and report any unusual or significant findings; contact the physician when necessary. Administer medication according to procedure if applicable. Initiate any and all emergency procedures as necessary. Conduct oversight of Charge Nurse and CNA performances. Contributes to and promotes positive resident and family relations. Other duties may be assigned by manager. Specific Requirements: Must be able to read, write, speak, and understand the English language. Must possess accurate and comprehensive assessment skills. Must have knowledge of restorative nursing and rehabilitation policies and utilizes appropriately. Must possess the ability to make independent decisions when circumstances warrant such action. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long term care. Must possess leadership and supervisory ability and the willingness to work harmoniously with professional and non-professional personnel. Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care. Participates in carrying out schedule of daily cleaning and disinfecting of the PACE center in accordance with CDC guidelines for increased precautions Actively participates in distribution of work for health aides to ensure care needs of participants and site tasks are completed timely and appropriately including but not limited to community-based care and center-based tasks, as assigned Actively participates in regular team meetings for health aides at respective site Participates in carrying out infection control precautions and increased cleaning and disinfecting of the PACE center as assigned and in accordance with current CDC guidelines Performs all duties in accordance with FH and Summit ElderCare policies and procedures. Qualifications Education: Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program is highly desirable, but we will accept equivalent work experience in lieu of certificate. Certification: Certification as a Home Health Aide or Nursing Assistant is desired. Reliable Transportation Required. Experience: One year of experience working with a frail and/or older adult population preferably in a PACE program. If under one year, must complete Summit Eldercare training program for health aides prior to working with participants independently. Must complete a standardized set of Summit Eldercare competencies for Health Aides on an annual basis.
