Looking for a dedicated, compassionate CHHA who will provide personal care services for our patients preparing for end-of-life. You will work under the direction of the Director of Nursing and Aides Manager. Keep basic computer documentation of personal care services rendered and other appropriate services on a daily. Patient documentation will be completed electronically within 24 hours of the visit. The qualified candidate will provide assistance and understanding to the family in time of bereavement. As a CHHA, you will work as a member of the Hospice team in providing Hospice care within the guidelines of the IDG care plan. These advanced technologies include, but are not limited to, a variety of high frequency ventilation devices, sophisticated neurological monitoring devices, Extra Corporeal Membrane Oxygenation (ECMO), Peritoneal Dialysis, and Continuous Renal Replacement Therapy (CRRT). CHOP Critical Care Nurses work collaboratively with the interdisciplinary team to ensure the best outcomes using innovative technologies. The Critical Care areas are equipped to deliver therapies that are aimed toward improving survival, hastening recovery, minimizing disability, and relieving pain and suffering from critical illness and injury in a safe, humane, and respectful manner. Conditions that commonly cause critical illness and injury or require intensive care include respiratory failure, renal failure, severe infection, multi-system organ failure, trauma, extensive surgery or organ transplantation, congenital anomalies, and hematologic or immunological disorders. Critical Care nurses across the Pediatric Intensive Care Unit are active participants in innovative unit-based research as well as organizational research initiatives. There are three distinct units within the Pediatric Intensive Care Unit (PICU). Unit populations are as follows: PICU South - 24 bed unit with a mixed medical and surgical population serving patients that require Intensive Care with a diagnosis within a variety of medical subspecialties including a specific subset of oncology receiving radioactive therapy. PICU Main - 26 bed unit with a mixed medical and surgical population serving patients that require Intensive Care with a diagnosis within a variety of medical subspecialties PICU West - 24 bed unit predominately focused on Neurocritical Care patients but also will care for all patients that would require pediatric intensive care. This is a full-time position; however, there may be opportunities for part-time based on unit needs at the time of application. Acute care experience (adult or pediatric) is strongly preferred. Applications to this requisition include all 3 PICU units. Candidates are hired to one primary unit and can be assigned a shift in another PICU unit based on the needs of the department. Job Description: Clinical Hospice Liaison Position Title: Clinical Hospice Liaison Position Summary: The Clinical Hospice Liaison serves as the clinical expert in the pre-admission process, assessing potential patients for hospice eligibility and coordinating a smooth and compassionate transition to hospice care. This position plays a crucial role in educating patients, families, and healthcare providers on the hospice benefit. Key Responsibilities: Perform clinical assessments of referred patients to determine hospice eligibility. Collaborate with physicians, case managers, and hospital discharge planners to coordinate timely referrals and admissions. Provide education to patients and families on the hospice philosophy, services, and expectations. Document all assessments, communication, and coordination efforts accurately and timely. Assist in completing necessary clinical and administrative documentation for admission. Serve as a clinical resource to the marketing and business development team. Attend interdisciplinary meetings and participate in community outreach as needed. Qualifications: Licensed Registered Nurse (RN) Minimum 2 years of clinical experience, preferably in hospice, palliative care, or acute care. Knowledge of Medicare hospice benefit and eligibility criteria. Strong communication and assessment skills. Ability to work independently and in collaboration with interdisciplinary teams. Valid driver's license and reliable transportation.
