In this role, the successful candidate The Licensed Practical Nurse (LPN) assists the Registered Nurse in the implementation of the nursing process on his/her assigned patients. The practice of nursing as an LPN is defined as performing tasks and responsibilities within the framework of case finding health teaching health counseling and provisions of supportive and restorative care under the direction of a Registered Professional Nurse. Job Responsibilities: Implements care in a knowledgeable, skillful, consistent and continuous manner, taking into consideration the age, growth and development of the individual. Demonstrates an awareness of and respect for coworkers/patient's cultural background Performs other duties as assigned or volunteered in alignment with the mission, goals and values of Lutheran Health Care. Provides translation services as needed. Demonstrates sound knowledge base and actions in the care and decision making for designated patients. Administers medication therapy and vaccines as ordered by the provider. Communicates patient care needs to ancillary staff and follow up accordingly Observes, reports and documents significant physical, psychological or safety status changes in patient's condition to the RN/PSC/Provider promptly. Provides health education based on patient's needs.Documents patient response to treatment/teaching in the medical record. Minimum Qualifications: To qualify you must have a Graduate of an accredited school of nursing. 1-2 years of experience in hospital based nursing, Med/Surg, Pediatrics, OB/Gyn. Ambulatory care experience preferred Willingness to devote the time required completing assigned tasks on schedule NYS LPN License Bilingual in languages based on site's needs Basic Life Support (BLS) Certification through the American Heart Association is required at the time of hire and is required to be maintained.. The Personal Care Aide is responsible for providing patient assistance with nutritional and environmental support and personal hygiene, feeding and dressing and/or, as an extension of a self-directed patient, selected health related tasks in his/her home or facility under the supervision of a registered nurse. Description RN/Case Manager Mount Sinai West Case Management FT Weekdays/Weekends/Holidays The Case Manager (CM) will be responsible for all aspects of case management for an assigned group of inpatients to determine the appropriateness of the admission and continued stay, assist in the development of the plan of care; ensure that the plan is implemented in a timely basis and identify the expected length of stay (ELOS). The CM works collaboratively with physicians, social workers, clinical nurses, home care services, and other members of the interdisciplinary team as needed. The CM actively participates in specific clinical initiatives focused on reducing the length of stay (LOS), improved efficiency, quality and resource utilization. Assignment will be by units/clinical areas of practice and may require responsibility on other units/services Responsibilities Principle Duties and responsibilities include, but are not limited to: Reviews all new admissions to identify patients where utilization review, discharge planning, and/or case management will be needed using standardized criteria to achieve optimal patient outcomes and appropriate reimbursement for the organization. Performs continued stay reviews utilizing standardized criteria to justify continued inpatient stay. Collaborates with Physician and other clinicians to expedite diagnostic testing, treatment and consultations. Documents all clinical reviews in Canopy. Supports the mission, vision, philosophy and goals of the Medical Center. Case Management: The CM process will include: Assessment of the patient’s clinical, psychosocial, and functional status in collaboration with the interdisciplinary team. Identification and documentation of variances affecting the LOS and the discharge planning process. Conducts follow-up of any delays in treatment or reporting of results. Planning/developing specific goals with the physician, interdisciplinary team, and the patient and/or family. Implementation and coordination of specific activities, strategies, and interventions to move the patient through the continuum of care. Documentation of outcomes achieved and identified internal and external barriers. Identifies reasons for readmissions and collaborates with interdisciplinary team on strategies to reduce readmission rate. Appropriately identifies and refers cases to the physician advisor to support timely progression of patients along the continuum of care and (appropriate) discharge planning. Interacts with patient/family to discuss plan of care and coordination of services based on clinical needs and available resources.
