Participates in health coaching, health fairs, coordinates biometric screenings, and other wellness activities. Assumes accountability for member satisfaction. Qualifications Required: Graduate of a state-approved School of Practical Nursing. Currently licensed as License Practical Nurse by the State of Ohio through the Ohio Board of Nursing. Current AHA Basic Life Support for the healthcare provider certification or obtains within 90 days of hire. 2 years of healthcare experience as an LPN, preferably in primary care, outpatient care, or care management. EMR knowledge and experience. Possesses a strong work ethic and ability to work independently and as a member of a multidisciplinary team. Displays a passion and strong commitment to delivering excellent customer service, with a preference for doing so in a healthcare setting. Knowledge of basic medical procedures and medical terminology. Excellent written, verbal, and interpersonal communication skills, and able to use those with a diverse patient population.
