Summary GENERAL SUMMARY: Responsible for facilitating access to Medical Center services. Secures medical necessity/diagnosis and patient demographic and financial information from physicians, physician staff, health care facilities, patients and patient representatives. Collects and analyzes pertinent clinical data as related to the procedure(s) scheduled, intervening to prevent problems that could result in adverse patient outcomes or delated/canceled procedures. Contributes to an efficient and effective work environment through activities including professional communication and documentation, preceptor and/or resourcing of others. PRINCIPAL JOB FUNCTIONS: 1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values. 2. *Performs multi-resource appointment scheduling for inpatient/outpatient diagnostic and therapeutic services received from physician and/or physician staff. Obtains patient information including medical necessity/diagnosis, demographics, and pertinent clinical information as related to the procedures scheduled. Collects and documents information beyond that listed in procedural questionnaire based on critical assessment of patient history. 3. *Integrates physical, mental and cultural considerations in scheduling of patient procedures, coordinating services to ensure the Medical Center is patient ready on customer arrival. 4. Educates patient/patient representative on preparation and understanding of procedures. 5. *Collaborates with physician office staff to educate the patient/patient representative on preparation and understanding of scheduled procedures; clearly communicates instructions to patient and/or patient representative. 6. Provides recommendations and education to physician office staff commensurate with scope of practice in scheduling of procedures. 7. *Reviews and evaluates outpatient orders received from the physician and/or physician staff; uses clinical judgment/critical thinking to follow-up on appropriateness and completeness of physician orders as they relate to procedures ordered. 8. *Receives and documents verbal physician orders. 9. Screens/determines appropriateness of level of care and admission status; notifies appropriate departments based on assessment of patient needs. 10. Documents pre-certification/authorization/referral information when applicable. 11. *Prepares and provides patients with an estimate, if one is warranted, for their expected services. Collects and/or counsels on expected payment due. 12. Keeps Patient Service Center and Scheduling Manager informed of issues related to procedures including but not limited to: necessary questionnaire revisions, updates related to departmental requests and need for new or revised procedures build. 13. *Serves as a liaison in building and maintaining professional relationships between physicians, physician office staff and other health care personnel. 14. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise. 15. Participates in meetings, committees and department projects as assigned. 16. Performs other related projects and duties as assigned. (Essential Job functions are marked with an asterisk “*”. Purpose of Position: Under the direction of the Clinical Manager/ RN Supervisor and within the Agency policies and procedures, the LPN follows the plan of care on individual patients within the guidelines of his/her licensure to provide for the safety and quality of care treatment or services. Description Assesses patient and family needs, reports new or pertinent findings to RN and physician. Follows prescribed plan of care as completed by the RN in order to address identified needs. Adheres to daily patient assignments as directed by the Clinical Manager/RN Supervisor providing personal care, observation, procedures, and medication administration. Demonstrates the skills and judgments to implement the medical plan of care, nursing interventions and procedures as necessary for the care of the patient, within the guidelines of his/her licensure. Assess, evaluates and reassesses the identified issues/symptoms patients are exhibiting, care provided and patient's response on a continuing basis. Meets the safety needs of the patient using equipment safely and properly. Collaborates and communicates on a regular basis with the RN/Clinical Manager and members of the interdisciplinary team as needed. Assists patients and families in accessing appropriate community resources. Records documentation to reflect patient care. Documentation is clear, concise and addresses specifics of teaching, direct care and response of patient / caregiver. Knowledgeable about and adheres to the VNA compliance plan, policies and procedures. Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers in a manner that reflects VNA’s commitment to CARES: compassion, accountability, respect, excellence and service. All other duties as assigned. Qualifications Graduate of an approved LPN school of nursing. Current LPN license in MA. CPR certification required. Purpose of Position: To provide professional nursing care and support for office patients who are receiving a treatment, examination, evaluation, etc. Through the nursing process of assessment and evaluation while maintaining standards of professional nursing within the realm of departmental policy. Description Under the direct supervision of the Physician or Operations/Practice Manager, assist in providing nursing care to office patients. Triage and evaluate patients, mainly via telephone calls and some walk-in patients; obtain major complaints (and brief medical history when necessary) and complete chart documentation Inform physician or nurse practitioner of any patient symptoms that you have addressed and/or for guidance on resolving any issues, if necessary. Obtain patient's current medication regimen, refill medication orders in the office or over the telephone under a physician's supervision and document same. Assist other departments as needs; i.e. Coumadin Clinic with Lovenox instructions to patients, out of range INR results, etc. Respond to Codes that are paged in the practice and assist with medical personal as needed. Answer general health questions for patients via telephone or in office with advice, referral, or appointment under physician's direction. Triage telephone calls with complete follow-through. Maintain, order and stock necessary pharmaceuticals weekly. Obtain verbal and written reports from outside agencies such as, VNA, radiology, or other physician's office with timely, concise transmittal to physician. To maintain patient rights and confidentiality of information. Serve as an advocate and provide a safe and secure environment. Participate in identification of the important aspects of care for each patient, the identification of indicators used to monitor the quality and appropriateness of care. Assess personal and learning developmental needs and act as a resource person. Participate in nursing and staff meetings and other activities which promote the growth and developmental of nursing and patient care. Assist with secretarial/clerical duties as needed including filing incoming paperwork, copying medical records, answering phones, taking messages and faxing. Perform other work related duties as requested/directed.
