Reports observations, changes, unusual conditions or reactions to supervisor or appropriate discipline in a timely manner. Takes appropriate action as requested or required by the situation. Notifies family of change in residents condition or hospitalization according to Masonic Village policy. Assist to identify resident needs and strengths. Assists in the development of the plan of care. Participates in resident care conferences as needed. Implements the plan of care and oversees care provided by assigned nursing personnel.
