Position Overview: The Trauma and Emergency General Surgery Advanced Practice Provider (APP) delivers direct patient care consistent with the APP’s education, experience, and certification, in collaboration with the attending Trauma and Emergency General Surgery physicians. This includes the diagnosis, management, and referral of health care issues specific to Trauma and Emergency General Surgery (EGS). The APP will complete pre- and post-surgical management and recognize options for non-surgical management of trauma, EGS, and critically ill patients. Key Responsibilities: · Collaborate with Trauma/EGS attending physicians, general surgery residents, and APPs to complete daily rounds on the trauma and surgical floors and in the intensive care unit (ICU). · Respond to Emergency Department (ED) consults for EGS, collecting data and interpreting laboratory and imaging results to develop and document assessments and care plans. · Perform bedside procedures for both surgical and non-surgical patients · Work with the team to complete timely documentation and carry out care plans effectively. · Partner with Trauma/EGS APPs, Trauma/EGS attendings, surgical residents, nursing, physical, occupational and speech therapies, social work and case management to develop disposition plans with a multidisciplinary team approach. · Respond to trauma cases in the resuscitation bay, perform trauma survey assessments, and stabilize high-acuity patients in accordance with Advanced Trauma Life Support (ATLS) guidelines. · Promote consumer, family, and community health through preventative health care actions and patient advocacy. · Provide pre-, intra-, and post-operative care, including periodically serving as a surgical first assistant during Trauma and EGS cases. · Rotate to our partnering Level 3 trauma center Fisher Titus in Norwalk to provide consistent and high-quality care to our surrounding communities. · Uphold the mission, vision, values, and customer service standards of The MetroHealth System. Trauma Advanced Practice Provider (APP) - Night Shift Position Overview: The Trauma Advanced Practice Provider (APP) delivers direct patient care consistent with the APP’s education, experience, and certification, in collaboration with the attending Trauma Surgery physicians. This includes the diagnosis, management, and referral of health care issues specific to Trauma. The Director of Telehealth is the single accountable owner of this program: responsible for building and managing the clinical team, redesigning the platform and workflows that support it, launching and overseeing CCM/BHI operations, and ensuring the program meets its coverage, quality, financial, and retention goals. This is a hands-on operational leadership role. The Director will directly supervise a rotating team of Nurse Practitioners/Physician Assistants (FTE and PRN) delivering after-hours and weekend telehealth coverage, chronic care management, and behavioral health integration services across CareMed's partnered skilled nursing, assisted living, and long-term care facilities. Key Responsibilities Program Build & Operations • Own end-to-end build-out and ongoing management of the telehealth program, including the clinical rotation schedule (weekday overnight and full weekend coverage). • Assess and restructure the current telehealth platform and clinical workflows to resolve existing fragmentation and inconsistent coverage. • Directly supervise the clinician team: scheduling, coaching, performance management, and clinical escalation support. • Serve as the primary liaison between the telehealth team, facility partners, and executive leadership on program performance. CCM & BHI Program Ownership • Own the Chronic Care Management (CCM) and Behavioral Health Integration (BHI) program build: patient eligibility screening, enrollment workflows, and patient consent. • Ensure documentation standards and billing accuracy for CCM (CPT 99490, 99439) and BHI (CPT 99484), including correct application of Medicare Part A stay exclusions and place-of-service rules. • Oversee accurate coding and documentation for after-hours telehealth encounters billed under Subsequent Nursing Facility Care codes (CPT 99307–99310). • Monitor CCM/BHI enrollment volume and encounter volume against revenue targets, and adjust outreach or staffing to close gaps. Quality, Compliance & Financial Accountability • Hold the team accountable to chart closure compliance (95%+ within 48 hours) and clinical audit/quality standards. • Track and report on avoidable hospitalization/ED reduction and 30-day readmission rates for the attributed patient population. • Own the program's financial performance: CCM/BHI and encounter revenue versus target, and the program's overall cost-to-revenue ratio. • Ensure all documentation and billing practices meet CMS and payer compliance standards; serve as the point of escalation for coding/compliance questions specific to telehealth, CCM, and BHI. Team Leadership & Retention • Own the retention strategy for the telehealth team: onboarding, workload distribution, escalation support, and culture. • Monitor and manage PRN utilization to ensure sustainable coverage without over-reliance on the backup pool. • Track team turnover and time-to-fill metrics, and proactively address drivers of attrition. Required Qualifications • Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in at least one of CareMed's operating states (OK, KY, WV, OH); multi-state licensure or eligibility strongly preferred. • Minimum 5 years of clinical experience in primary care, geriatrics, long-term care, or a related acute/post- acute setting. • Minimum 2–3 years in a clinical leadership, supervisory, or program management role overseeing other clinicians. • Direct experience with telehealth service delivery, including platform-based virtual care in skilled nursing, assisted living, or similar settings. • Working knowledge of CMS billing and documentation requirements for Chronic Care Management, Behavioral Health Integration, and Transitional Care Management. • Demonstrated ability to manage a rotating, multi-shift clinical schedule (including nights and weekends) across multiple states or regions. Preferred Qualifications • Prior experience launching or scaling a CCM or BHI program from the ground up. • Familiarity with Subsequent Nursing Facility Care coding (CPT 99307–99310) and place-of-service billing rules for facility-based populations. • Experience with EMR/telehealth platform selection, configuration, or workflow redesign. • Background in value-based care, quality metrics (QAPI, MDS, star ratings), or population health management. • Experience managing PRN/float staffing pools and rotating shift schedules in a healthcare setting.
