In this role, the successful candidate will be responsible for the provision, coordination, and assurance of comprehensive nursing care using nursing process and pertinent standards of care, the diagnosis of illness and physical conditions, and performance of therapeutic, corrective, and prescriptive measures of the designated inpatient or outpatient patient population within a collaborative practice agreement established with a specific physician on the staff of NYU Hospitals Center Job Responsibilities: Plans and organizes care to meet individual patient needs and to ensure appropriate clinical resource utilization per protocols, pathways, and other means. Monitors and trends the cost effectiveness of the position, particularly as it relates to the efficiency and performance indicators established for the service. Assists in the preparation, implementation, and evaluation of research protocols when applicable. Works collaboratively with nursing and other disciplines in the development and implementation of clinical studies within the area of expertise. Uses evidence-based health care literature to advise and support appropriate practice changes within the designated service. Participates in the development and monitoring of patient outcomes per established practice protocols for purposes of quality and performance improvement. Serves as a clinical expert and resource for the education of peers and other health professionals. Maintains current expertise in area of practice. Promotes own professional growth and development in clinical and managerial role. Considers needs and behaviors of specific patient age and cultural groups in all patient care. Prescribes medical equipment, devices, physical and occupational therapy, and home health services per practice protocol. Prescribes in-patient and discharge medications according to New York State law, including controlled substances with DEA authorization. Works collaboratively with the attending physician, consulting physician(s), and other disciplines to identify, develop, and implement an appropriate plan of care that maximizes individual patient/family preference and enhances quality, access, and cost-effective outcomes. Formulates the plan of care, along with the attending physician, patient and family, based on expected goals of care and length of stay. Orders customary laboratory, radiological and diagnostic studies per practice protocols, differentiates between normal/abnormal findings and follows protocols for abnormal findings, which may include referral to consulting physicians per customary pattern of the attending MD or with the collaborating physician. Performs complete health history, physical examination and psychosocial assessment of patients within the designated practice or clinical unit. Functions as a respectful member of the health care team. Participates in designated activities related to professional nursing standards and regulations of relevant outside agencies. Demonstrates knowledge of the organizations Service Standards and incorporates them into the performance of responsibilities. Facilitates the continuity of care across the continuum of services, e.g. home care, other facilities, appointments with MDs etc. Provides consultation about the therapeutic regimen to the nursing staff responsible for the patients care in the hospital setting. Minimum Qualifications: To qualify you must have: Licensure: Current registration as Professional Nurse in New York and Registration as Nurse Practitioner in New York. Education: Master's Degree in Nursing. Certification required (as soon as feasible following graduation from NP graduate program) by ANCC or Board Certification from other specialty organization that certifies Nurse Practitioners in a clinical area of practice. Join the University of Chicago Medicine as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) for our Oncology Rapid Assessment Clinic (ORAC). The Advanced Practice Provider will be responsible for comprehensive direct patient care management and will manage patients by exercising autonomous decision making in the assessment, diagnosis, and initiation of delegated medical treatments. The APP will provide healthcare and patient services that may include educational, research, and clinical support, as well as administrative activities under the direction and supervision of the Cancer Service Line leadership. The APP has acquired the knowledge, skills, and judgment necessary to deliver high quality care to hematology/oncology patients. The Oncology Rapid Assessment Clinic (ORAC) i s an APP-staffed clinic area that cares for cancer patients who require urgent, same day/next day assessment and treatment. Hematology Oncology often require acute care for symptoms of malignancy as well as for the toxicities and side effects of cancer treatments. After an orientation period, the APP will work autonomously and collaboratively with the rest of the ORAC APP team. We are seeking dedicated professionals who share our passion for serving those who have served our country. ________________________________________ POSITION SUMMARY Primary Care Solutions is seeking an experienced and highly motivated PRN Nurse Practitioner to provide short-term coverage at VA Community-Based Outpatient Clinics (CBOCs) in multiple states where PCS operates. This position offers exceptional scheduling flexibility, allowing providers to work anywhere from one week per month to full-time equivalent coverage, depending on clinic needs and provider availability. Assignments are generally Monday through Friday, with travel occurring on Sunday and return travel on Friday evening or Saturday, depending on location and flight availability. This is a fully on-site, patient-facing position providing comprehensive primary care services to our nation's Veterans. Responsibilities include: • Conduct comprehensive patient assessments and examinations • Diagnose, treat, and manage acute and chronic medical conditions • Develop and implement evidence-based treatment plans • Review laboratory, imaging, consultative, and diagnostic information to guide patient care • Complete preventive health screenings and VA clinical reminders • Coordinate care with interdisciplinary Patient Aligned Care Team (PACT) members • Document encounters accurately and timely within the VA electronic health record system (VISTA/CPRS) • Ensure compliance with VA clinical standards, policies, and performance measures • Promote positive patient experiences and outstanding clinical outcomes ________________________________________ REQUIRED QUALIFICATIONS • Graduate of an accredited School of Nursing • Graduate of an accredited Nurse Practitioner program (MSN required) • Board Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), or other applicable primary care specialty • Current certification through an accredited certifying organization • Minimum one year of clinical practice experience preferred • Active, unrestricted state nursing licenses in states where services are provided • Active, unrestricted DEA registration for applicable practice locations • Current American Heart Association Basic Life Support (BLS) certification • Strong computer and documentation skills • Ability to travel regularly and work independently in various clinic locations ________________________________________ CREDENTIALING REQUIREMENTS Selected candidates must: • Successfully pass all required federal background investigations • Meet VA credentialing and privileging requirements • Obtain and maintain active credentialing with each VA Medical Center (VAMC) where services are performed • Maintain all required licenses, certifications, registrations, and continuing education requirements throughout employment ________________________________________ SECURITY AND CONFIDENTIALITY This position requires access to confidential and sensitive information.
