Identifies and appropriately refers cases to Child/Adult Protective Services, ensuring compliance with legal and ethical standards. Provides professional assistance to patients, families, and physicians regarding discharge planning and post-hospital care options. Performs other duties as assigned. Qualifications Bachelor's Degree in Nursing preferred 2-4 years of clinical nursing experience in a hospital, home health, or nursing home setting required 2-4 years of care management experience preferred Knowledge, Skills and Abilities Strong understanding of case management principles, discharge planning, and transitions of care. Knowledge of federal, state, and Joint Commission standards related to case management. Excellent communication and interpersonal skills to collaborate effectively with patients, families, and interdisciplinary teams. Ability to assess complex situations, identify solutions, and implement care plans efficiently. Proficiency in electronic medical records (EMR) and documentation systems. We take patients with ischemic and hemorrhagic strokes, brain tumors, aneurysms, seizures, traumatic brain injuries, neuromuscular diseases and other diagnoses involving the neurologic system. The floor beds consist of patients requiring every 4-hour monitoring. Additionally, there are 7 Epilepsy Monitoring Unit (EMU) beds embedded in the 21 neuro floor beds. These patients are seizure disorder patients admitted electively with their anti-convulsant medications on hold and hard wired to continuous EEG. This allows the physicians to identify the area of the brain where the seizures are occurring. Many of these patients become candidates for brain mapping with surgically placed grids to further pinpoint the area causing the seizures. This mapping allows the surgeon to determine if the patient would be a candidate for surgical removal of parts of the brain to stop seizure activity and still allow the patient to have functionality. The nurse-to-patient ratio for floor patients is typically 1:5. Provides preop, intraop, and post-op care, including assessment and discharge instructions. Assess the patient’s condition before, during, and after the operation to ensure an optimal outcome for the patient. Anticipates the scrub nurse’s needs and opens sterile packs, operates equipment, and keeps accurate records. Responsible for reprocessing if on Endoscopy team.
