Maintains an attractive and comfortable environment for patients with special consideration to cleanliness of room, ventilation, and lighting. Assessment visits generally last 30–60 minutes plus prep work, travel time, and mileage all of which is paid Travel to clients' homes within the assigned service area is required. Conduct comprehensive, person-centered assessments. Accurately identify each client's home care needs and service recommendations. Communicate assessment findings promptly to the Care Coordinator to support timely care planning. Represent the agency with professionalism, compassion, integrity, and respect during every interaction with clients, families, caregivers, and referral sources.
