Nurse Practitioner - Complex Care Services Department Summary Advanced Practice Registered Nurse (APRN) to function in the inpatient Complex Care Service (CCS) at Boston Children’s Hospital. CCS is a long-standing and nationally regarded inpatient service in Pediatric Hospital Medicine within the Division of General Pediatrics that serves hospitalized children with medical complexity, who have complex chronic conditions and functional limitations. Common reasons for admission to CCS include infections (aspiration pneumonia, pyelonephritis), chronic condition exacerbations (epilepsy, digestive dysmotility), and post-operative recovery following surgery (baclofen pump placement, gastrostomy, spinal fusion, etc.). This role will provide patient evaluation, assessment, and facilitate care for the patients on the CCS service and will implement interventions tailored to improve systems for patient care and enhance patient safety for this population. The CCS APRN will work collaboratively with supervising physicians, pediatric trainees (residents and fellows), and the entire nursing team to promote optimal clinical care. This APRN will act as a clinical resource, facilitating staff educational opportunities. The setting is fast-paced and collaborative with many opportunities for learning. Requirements include completion of a master’s degree program in Nursing. Clinical experience with children with medical complexity is preferred. Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident’s response to the care. Fill out and complete accident/incident reports. Submit to Director as required. Chart all reports of accidents/incidents involving residents. Follow established procedures. Record new/changed diet orders. Forward information to the Dietary Department. Report all discrepancies noted concerning physician’s orders, diet change, charting error, etc., to the Nurse Supervisor. Fill out and complete transfer forms in accordance with established procedures. Ensure that appropriate documentation concerning unauthorized discharges is entered in the resident’s medical record in accordance with established procedures. Perform routine charting duties as required and in accordance with established charting and documentation policies and procedures. Sign and date all entries made in the resident’s medical record. Drug Administration Functions Prepare and administer medications as ordered by the physician. Verify the identity of the resident before administering the medication/treatment. Ensure that prescribed medication for one resident is not administered to another. Ensure that an adequate supply of floor stock medications, supplies, and equipment is on hand to meet the nursing needs of the residents. Report needs to the Nurse Supervisor. Order prescribed medications, supplies, and equipment as necessary, and in accordance with established policies. Ensure that narcotic records are accurate for your shift. Notify the Nurse Supervisor of all drug and narcotic discrepancies noted on your shift. Review medication cards for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Notify the attending physician of automatic stop orders prior to the last dosage being administered. Dispose of drugs and narcotics as required, and in accordance with established procedures. Meet with your shift’s nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or to improve services. Ensure that department personnel, residents, and visitors follow the department’s established policies and procedures at all times. Develop and maintain a good working rapport with inter-departmental personnel, as well as other departments within the facility to ensure that nursing services and activities can be adequately maintained to meet the needs of the residents. Review complaints and grievances made or filed by your assigned personnel. Make appropriate reports to the Nurse Supervisor as required or as may be necessary. Nursing Care Functions Inform nursing personnel of new admissions, their expected time of arrival, room assignment, etc. Ensure that rooms are ready for new admissions. Participate in the orientation of new residents/family members to the facility. Make rounds with physicians as necessary. Requisition and arrange for diagnostic and therapeutic services, as ordered by the physician, and in accordance with our established procedures. Consult with the resident’s physician in providing the resident’s care, treatment, rehabilitation, etc., as necessary. Review the resident’s chart for specific treatments, medication orders, diets, etc., as necessary. Implement and maintain established nursing objectives and standards in accordance with the plan of care. Make periodic checks to ensure that certified nursing assistants are properly administering prescribed treatments and to evaluate the resident’s physical and emotional status. Cooperate with the coordinate social and activity programs with nursing service schedules. Notify the resident’s attending physician and next-of-kin when there is a change in the resident’s condition. Carry out restorative and rehabilitative programs, to include self-help and care. Inspect the nursing service treatment areas daily to ensure that they are maintained in a clean and safe manner. Administer professional services such as; catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking blood, giving massages and range of motion exercises, care for the dead/dying, etc., as required and authorized within the scope of practice for LPNs as defined by the Pennsylvania Nurse Practice Act . Use restraints when necessary and in accordance with established policies and procedures in accordance with the plan of care. Obtain sputum, urine and other specimens for lab tests as ordered. Take and record TPRs, blood pressures, etc., as necessary. Monitor seriously ill residents as necessary. Check foods brought into the facility by the resident’s family/visitors to ensure that it is within the resident’s dietary allowances. Report problem areas to the Nurse Supervisor and Dietary Supervisor. Ensure that personnel providing direct care to residents are providing such care in accordance with the resident’s care plan and wishes. Ensure that residents who are unable to call for help are checked frequently. Meet with residents, and/or family members, as necessary. Report problem areas to the Nurse Supervisor. Admit, transfer and discharge residents as necessary. Assist in arranging transportation for discharged residents as necessary. Ensure that discharged residents are escorted to the pick-up area. Inform family members of the death of the resident. Call funeral homes when requested by the family. Ensure that established post-mortem procedures are followed. Staff Development Participate in developing, planning, conducting, and in-service training classes that provide instructions on “how to do the job,” and ensure a well-educated nursing service department. Implement and maintain an effective orientation program that orients the new employee to your shift, its policies and procedures, and to his/her job position and duties. Assist in standardizing the methods in which work will be accomplished. Assist in training department personnel in identifying tasks that involve potential exposure to blood/body fluids. Attend and participate in outside training programs. Attend and participate in annual OSHA and CDC in-service training programs for hazard communication, TB management, and blood borne pathogens standard. Attend and participate in continuing education programs designed to keep you abreast of changes in your profession, as well as to maintain your license on a current status. Safety and Sanitation Monitor your assigned personnel to ensure that they are following established safety regulations in the use of equipment and supplies. Ensure that your assigned nursing personnel follow established departmental policies and procedures, including dress codes. Assist the Director and/or Infection Control Coordinator in identifying, evaluating, and classifying routine and job-related functions to ensure that tasks in which there is potential exposure to blood/body fluids are properly identified and recorded. Ensure that all personnel performing tasks that involve potential exposure to blood/body fluids participate in appropriate in-service training programs prior to performing such tasks. Hospice Nurse Practitioner

Overview The Nurse Practitioner will serve in a dual Hospice and Palliative Care role. In this position, they will provide comprehensive, patient-centered care across the continuum of serious illness, supporting patients in both palliative and hospice settings. The role includes a blend of hospice and palliative responsibilities with flexibility based on patient census and program needs. The Nurse Practitioner will deliver advanced clinical care, perform patient management visits and hospice Face-to-Face (F2F) encounters for eligibility certification and recertification, and partner closely with physicians and interdisciplinary teams to ensure high-quality, compliant, and compassionate care. This role blends autonomy with collaboration, allowing you to guide patients and families through complex medical decision-making, symptom management, and end-of-life care. Compensation may vary within the salary range provided based on several factors including but not limited to a candidate’s location, experience, education, skills, licensure, certifications and department equity. Salary Range – $75.00/hour - $85.00/hour Essential Functions Essential Functions of Position Clinical Management Complete all visit types for patients on services, including Medicare-required hospice Face-to-Face (F2F) visits within CMS established regulatory timeframes, History & Physical/Initial Comprehensive visits, Problem-focused Symptom Management visits, Admission visits, Goals of Care discussions, and routine follow up visits. Collaborate with Hospice and Palliative Medical Directors and interdisciplinary teams to deliver comprehensive, patient-centered care. Collaborate with Primary Care Provider (PCP)/Attending Provider and specialists (if applicable) to coordinate patient Plan of Care Perform in-depth patient assessments, including physical, emotional, functional, and psychosocial evaluations. Support individualized care plans in partnership with the patients, families, caregivers, and IDG/IDT. Assist with and/or manage all aspects of patient care including diagnosis, acute and chronic symptoms, treatment, medication management, assessments, longitudinal serious illness support, quality-of-life optimization, and consultations, while reducing avoidable or unnecessary acute care utilization. Prescribe medications and treatments, including controlled substances, as permitted by state law and federal regulation and within defined parameters of Independent Practice or a Collaborative Practice Agreement. Order and interpret laboratory and other diagnostic tests within the state defined scope of practice and Collaborative Practice Agreement. Facilitate goals-of-care discussions, advance care planning, and level of care transitions when appropriate. Coordinate care across settings, including home, skilled nursing facilities, and assisted living. Initiate referrals to supportive services and community resources as needed. Utilize Local Coverage Determination (LCD) criteria to support hospice eligibility and recertification. Document clinical findings accurately and attest F2F encounters in the medical record in compliance with regulatory standards. Communicate findings, treatment plans, diagnostics, and medication reviews with IDG/IDT. Provide clinical recommendations to the interdisciplinary group (IDG) based on assessment findings. Ensure alignment with Medicare, Medicaid, and accrediting body requirements (e.g., ACHC). Notify appropriate team members promptly of any barriers to completing scheduled visits. Provide patient care and visit coverage across multiple provider numbers, as needed and approved. Maintain comprehensive0 knowledge in the field of hospice and palliative care. Collaboration & Communication Partner effectively with physicians, nurses, social workers, chaplains, and other IDG/IDT members. Communicate clearly and compassionately with patients, caregivers, and families. Support patients and families through serious illness and end-of-life transitions with empathy and clinical expertise. Participate in on-call rotation as needed. Documentation & Compliance Maintain timely, accurate, and compliant clinical documentation. Adhere to all federal, state, and organizational regulations, including OSHA and hospice Conditions of Participation. Demonstrate understanding of hospice eligibility, documentation standards, and Part B billing practices (as applicable). About You To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Education/Experience: Master of Science in Nursing (MSN) or higher from an accredited program. Completion of an accredited Nurse Practitioner program. Minimum of two years of Nurse Practitioner experience preferred. Experience in hospice, palliative care, oncology, geriatrics, family/adult medicine, home health, or acute care preferred. Experience with Part B billing preferred. Licenses, Certifications and/or Registration: Current RN license and Nurse Practitioner license/certification in state of practice. National NP certification (ANCC or AANP). Collaborative Practice Agreement with supervising internal physician or Proof of Independent Practice Authority in accordance with state regulations. Controlled Substance Agreement, if applicable. DEA registration and prescriptive authority in accordance with state regulations. Current CPR certification. ACHPN certification preferred. Valid driver’s license and current automobile insurance. Specialized Knowledge and Skills: Strong clinical assessment and critical thinking skills across palliative and hospice populations. In-depth understanding of palliative care principles, hospice philosophy, home health eligibility, and end-of-life care. Working knowledge of LCD criteria, Medicare/Medicaid regulations, coding/billing, and documentation requirements. Excellent interpersonal, written, and verbal communication skills. Knowledgeable of Hospice & Palliative Quality Measures collection, documentation, reporting, and the QAPI process. Ability to work both autonomously and collaboratively in a fast-paced, evolving environment. Compassionate, emotionally intelligent, and sensitive to the needs of patients and families. Proficient in point-of-care technology and electronic medical records. Willingness and ability to maintain productivity and documentation standards in alignment with organizational expectations.
