The Behavioral Health Nurse is responsible for carrying out duties as a health coach as well as utilizing skills as a psychiatric nurse in various care areas when needed, including but not limited to the Intensive Outpatient Program and Psych Triage in the Emergency Department. What you will Do: Assesses and evaluates individuals' physical, psychosocial, environmental, spiritual, social and education needs. Works collaboratively with the Behavioral Health inpatient and outpatient teams to achieve desired patient care outcomes and ensure a smooth transition of care Provides, facilitates, and/or implements learning opportunities to meet the educational needs of patients, families or members of the community.
