The Home Health Travel RN will travel extensively throughout our markets to provide, coordinate, and direct home health nursing care according to physician orders and agency policies. Primary Responsibilities: Assume responsibility for assigned patient's care from admission through discharge by effectively executing patient's Plan of Care. Collaborate with the patient, family/caregiver(s), ordering physicians, other care team members, and community resources. Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes. Participate in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals. Qualifications: Strong home health knowledge, and recent experience (2 years+) working in the home health industry. In-depth understanding of Medicare Conditions of Participation and payment mechanisms for home health. Strong organizational, planning, problem solving, decision-making, and leadership skills required. Strong interpersonal skills. Ability to communicate effectively in writing and verbally with people who have varied background. Ability to direct and manage a multi-disciplinary team of home health care clinicians, both professional and para-professional. Proof of current CPR certification and valid Driver’s license. Requirements: Registered Nurse with at least 2 years of recent home health experience. Knowledge of HCHB is strongly preferred. Bachelor’s degree in nursing is preferred but not required. License and current registration to practice in related discipline in the states where the company is located – MO, OH, MI, IL, KS Computer skills and ability to use word processing, database, spreadsheet, applications, etc. Ability to travel up to 100% of the time. Pediatric or GI experience preferred. Schedule: 8:00 am - 4:30 pm , no on call Job Summary: Provides services as a patient navigator throughout the continuum of care. Job Responsibilities and Requirements: PRIMARY RESPONSIBILITIES Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers. Collaborates with physicians and advanced practice providers to ensure quality outcomes. Coordinates efforts for treatment and promotes quality improvement and educational efforts. Compiles data, tracks outcomes, and makes recommendations for process improvement. Provides referrals/support for follow-up care for patients who are uninsured or underinsured. Utilizes the nursing process of assessment, planning, intervention, and evaluation in all patient encounters. Assists medical provider with processing of diagnostic results bringing significant values to the provider’s attention. Exhibits knowledge of tests and procedures and their significance as related to patient care. Recognizes the implementation of dosages, interactions, side effects, adverse effects, routes of administration of drugs as they apply to individual patients. Uses knowledge of medications in instructing patients about medication therapy. Establishes working relationships teaching protocols for patient education, family counseling and general public information. Works in a constant state of alertness and safe manner. Performs other duties as assigned. EDUCATION Graduate of accredited school of nursing or education equivalency for licensing EXPERIENCE Three years' registered nurse experience PHYSICAL REQUIREMENTS Constant use of speech to share information through oral communication. Constant standing and walking. Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs. Frequent sitting, reaching and keyboard use/data entry. Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors. Frequent use of smell to detect/recognize odors. Frequent use of hearing to receive oral communication, distinguish body sounds and/or hear alarms, malfunctioning machinery, etc. Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs. Occasional lifting/moving of patients. Occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive foot/leg and hand/arm movements. Occasional driving. Rare crawling and running.
