Ensures optimal pain control and patient comfort; identifies and discusses patient anxieties, fears or concerns regarding patient condition, treatment or discharge. Ensures that patient understands medication purpose, side effects, and administration instructions in the hospital as well as at the time of discharge . Documentation: Charting is accurate, legible, dated, and timed. Documentation reflects nursing process and interventions and evaluations taken. Utilizes computer systems effectively and efficiently for optimal patient care. Clinical Outcomes: Discusses patient findings and progress toward outcomes with physicians and other members of the health care team. Demonstrates competencies during the probationary period and ongoing completion by departmental competency validation. Make comprehensive nursing decisions based on interpretation of data, assessments, and evaluations of patient outcomes. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice. 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. We are seeking a dedicated Afterhours RN Care Manager to provide high-quality, compassionate hospice care outside standard business hours. This role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours. About the Role The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient’s plan of care. This role serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team. Clinical administrative responsibilities, workflow auditing, and survey readiness support are also part of this role. Essential Functions Ensure appropriate delivery of hospice services to eligible patients and families. Provide direct in-person patient care during on-call, weekend, and after-hours shifts, including assessments, admissions, routine visits, death visits, and live discharges. Conduct initial and ongoing assessments of physical, emotional, psychosocial, and spiritual needs. Support the IDG process through chart reviews, plan of care updates, and interdisciplinary communication. Develop and revise the Hospice Plan of Care in collaboration with physicians and IDG members. Educate patients and families on disease progression, symptom management, medications, and hospice philosophy. Communicate significant changes in patient condition promptly to hospice and attending physicians. Maintain accurate, timely clinical documentation including assessments, care plans, and notes. Assist with rescheduled, missed, or reassigned visit requests based on triage direction. Participate in calendar and scheduling modifications, including service codes and clinician assignments. Support administrative and compliance activities including workflow audits, survey readiness tasks, and documentation collection. Assist with TPE reviews, QAPI efforts, and regulatory compliance. Ensure infection control standards and universal precautions are consistently followed. Provide leadership and guidance to field staff; conduct supervisory visits when appropriate. Participate in required meetings, after-hours updates, and trainings. Maintain proficiency in pain and symptom management for hospice and end-of-life care. Summary: The Registered Nurse is responsible for the delivery of patient care and teaching through the nursing process. Demonstrates knowledge and skill in nursing theory and practice of the nursing process. Nurses assigned to a specialty unit will be required to meet the criteria established for that unit. ED RN - Weekends Only

We are excited to be offering a Weekend Only Program for RNs! Eligibility: RNs in inpatient nursing settings with 24/7 scheduling are included. 0.9 FTE would work two 12-hour shifts on the weekend and one additional shift during the week. Compensation: RNs will receive an additional $10 per hour plus their base pay. If the RN is a full-time (3)12s a week RN, the stipend will apply to the two shifts on the weekend and not on the third weekday shift. This $10 per hour will be combined with shift differentials as well as other special codes. Scheduling: RNs will be required to work 48 out of 52 weekends per calendar year. Weekends are defined as Friday and Saturday night for night shift. RNs can request up to 48 hours of PTO on their weekend shifts as well as 48 hours of exchanging a weekend shift for a weekday shift, for a total of 96 hours ‘off’ of the weekend requirement. This would be given in accordance with unit guidelines with allotted PTO as well as peak times, etc. RNs may request 1 holiday off per calendar year that falls on the weekend. The RN would only work the holiday if it fell on the weekend, they will not be moved off a weekend to work on a holiday. About the unit Greenville Emergency Department is a community-based hospital that is part of the Corewell Healthcare System. The Greenville ED is a certified Primary Stroke Center and Level IIII Trauma Center. There are over 25 care spaces to treat patients with minor to major medical and traumatic emergencies. Interdisciplinary team members collaborate closely to provide relationship-based care for our patients and their loved ones.
