Job Description Summary: An Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home, or in an alternate infusion suite, to ensure patient safety with continuity and compliance under a physician’s plan of care. Works under structured supervision of the designated supervisor. Job Description: ​ Job Responsibilities (listed in order of importance and/or time spent) Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals. Organizes and participates in the provision of direct patient care, performs treatments, administers medications, and educates patients and families/caregivers. Modifies plan of treatment in response to changing patient status or physician orders to achieve established or revised patient care goals. Assesses patient needs and physical status at each skilled visit through health data access and patient interview. Re-evaluates patient needs through physical reassessment, response to therapy, and supplemental physician orders. Obtains and clarifies physician orders for plan of treatment revisions, informs physician promptly of significant changes in patient's condition, and provides written summary to physician within supplemental order. Provides training and mentorship to Infusion Nurse I’s as needed to ensure patient safety and compliance. Provides effective and safe teaching using patient-centered care approach for patient and family to achieve independence with prescribed therapy and care needs through active participation per plan of treatment goals. Effectively and timely communicates with Option Care’s Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepares clinical documentation in real-time during visits and submits to the nursing department to comply with established timelines for billing optimization. Completes all documentation legibly and applies approved abbreviations and documentation error correction practices per Option Care’s policy. Coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care. Demonstrates compliance with agency operations, Option Care’s policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards. Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicates patient care provided, and follows assigned visit schedule. Responds promptly and appropriately to patient requests. Initiates emergency procedures as necessary. Accepts accountability for own practice through ethical and professional conduct. Follows established programs and practice within policies and procedures reflective of Option Care’s mission, values, and objectives. Observes legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary Option Care information including adherence to HIPAA regulations. Speaks knowledgeably about Option Care’s scope of services and effectively instructs patients about related financial obligations for care and service charges. Attends and completes required training modules, in-services, and continuing education to maintain competency and professional licensure for demonstrated knowledge regarding the care and management of patients in the home and/or alternate care settings. Effectively provides oversight and coordination of paraprofessionals in the home setting and may act as the Supervisor in the absence of the Supervisor or Nurse Manager as applicable in accordance with state and federal regulations. Participates in multidisciplinary team conferences and provides precepting, training, and mentoring to other nurses for orientation, and onboarding and supervisory activities as assigned. Participates in nursing department on-call responsibilities. Performs other related duties as directed by supervisor. If applicable: Inserts PICC/Midlines per physician order. The Universal RN (Registered Nurse) provides skilled nursing care in accordance with InnovAge policies, clinical standards, and all applicable regulations. This universal role supports participants in both clinic and in-home settings (as assigned), partners with the interdisciplinary team (IDT) to develop and execute individualized Plans of Care, and supports safe, efficient care delivery for frail and elderly participants. Physical presence at an InnovAge center and/or participant home is essential to performing the duties of this role. Essential Functions & Responsibilities Clinical Care & Care Coordination Perform comprehensive assessments, reassessments, and ongoing monitoring to identify needs, changes in condition, and appropriate level of care; update Plans of Care accordingly. Implement provider orders (MD/DO/NP/PA), including administering medications, treatments, wound care, and other skilled nursing interventions. Triage participant and staff calls to determine priority and route/escalate appropriately. Provide medication administration, reconciliation, and training in self-administration for non-scheduled medications; monitor and promote adherence. Educate participants and caregivers/families on conditions, medications, equipment, safety, and disease management. Schedule and coordinate outside specialist visits, diagnostics, and hospital admissions as needed; arrange transportation when applicable. Observe, record, and promptly report participant condition and response to treatments/medications to providers. Participate in IDT meetings, family conferences, and case reviews; contribute to the development and adjustment of Plans of Care. Complete timely, accurate documentation in the medical record; meet all agency and regulatory charting, reporting, certification/recertification, and discharge summary requirements. Support utilization review, peer review, and quality management activities as assigned. Setting-Specific Responsibilities: Clinic (Center-Based): Prepare, stock, and maintain exam and triage rooms; ensure availability of required supplies and materials. Assist with participant examinations, testing, and treatments performed in the clinic. In the absence of the Nursing Services Director/Clinic Nurse Supervisor, supervise clinic staff and support overall clinic workflow. In-Home Services: Provide skilled nursing care in the participant’s place of residence; deliver preventive, restorative, and rehabilitative nursing procedures. Determine type and amount of nursing care needed for each participant and coordinate with Home Care resources. Supervise LPNs at least every 30 days and HHAs/CNAs at least every 14 days (or per regulatory requirements); provide teaching and competency reinforcement. Supervises unskilled care every 90 days. Conduct supervisory visits, care conferences, and periodic participant/caregiver check-ins; trend data and collaborate with the Home Care Manager and Nursing Services Director to ensure quality care delivery. Safety, Compliance & Operations Adhere to InnovAge policies and all applicable federal, state, and local regulations (e.g., HIPAA, OSHA, infection prevention, medication safety, scope of practice). Use PPE and follow standard, transmission-based, and universal precautions. Participate in on-call, after-hours, and holiday coverage as assigned. Support continuous improvement, patient safety initiatives, and cost stewardship. Comfort Care • Develop EOL (end of life) documentation, forms, standards, and protocols • Conduct initial EOL assessments of participants to determine their individual needs and coordinate a plan of care. • Provide decision-makers with care options and resources to aid them in making difficult decisions in the best interest of the participant. • Educate family members about the end-of-life process and ensure they receive the "Caregiving at Life's End" manual. • Provide psychosocial support to participants and their families, including coping and bereavement resources in collaboration with the Social Work department. • Inquire about participants' and families' spiritual needs and call upon appropriate resources. Current First Aid and BLS certifications from American Heart Association and/or American Red Cross (required prior to hire). 1+ year experience working with frail or elderly populations. For assignments that include in-home care: personal transportation, valid state driver’s license, acceptable driving record, and auto insurance as required by law. Preferred Bachelor’s degree in nursing Certification as a Gerontological Nurse Minimum of 2 years of health care experience with emphasis in geriatrics, with at least one year being an EGL/Hospice. Experience and knowledge of supportive/bereavement counseling techniques, strategies and available resources and the ability to apply/inform family members and/or caregivers. Experience with conducting end of life assessments and developing appropriate plans of care (POC).
