Registered Nurse Assessment Coordinator (RNAC)

The RAI Coordinator is responsible for the timely completion of all OBRA and Medicare assessments to comply with federal regulations. Oversees the overall process and tracking of Prospective Payment System (PPS) Minimum Data Set (MDS) assessments including completion, submission and correction as needed. Integrates nursing, dietitian, therapeutic recreation, restorative, rehabilitation, and physician services to ensure accurate assessments which capture all clinical conditions and provided services. · Confirms payer source of all residents per HIQA. · Ensures the accurate and timely completion of all MDS Assessments including OBRA and Medicare PPS. · Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident’s stay. · Responsible for electronic submission of all MDS assessments. Verifies electronic submission of MDS and performs corrections when necessary and maintains appropriate records. · Coordinates interdisciplinary participation in completing the MDS for each resident according to regulatory time frames. Ensures completeness of MDS and thoroughness of documentation as mandated by federal and state standards. · Schedules resident care conferences in compliance with state and federal regulations and ensures completion of all MDS reviews prior to resident care conference. · Assists disciplines with care planning process by ensuring the MDS information is completed timely, the Care Area Assessments (CAAs) are analyzed thoroughly, and the team has completed the MDS prior to care conference. · Ensures all Medicare documentation follows federal and state guidelines and is completed timely. This includes the physician certification/Recertification, Traditional Medicare and Medicare Advantage non-coverage letters for part A and B. · Monitors activities of daily living for accuracy and provides education when warranted. · Conducts daily PPS meeting, weekly Medicare/Insurance and Quarterly Clinical Review meetings. · Coordinates the monthly billing process including the triple check meeting, the attested billing report, and the HMO log. · Assists with the appeal process to insure that timelines are met and appeals submitted . · Coordinates managed care process including submitting timely updates and obtaining ongoing authorizations. · Ensures the timely completion of Medicaid Picture Date submissions.

In the pulmonary suite (STU only), endoscopy of the airway procedures is performed, and are commonly called bronchoscopy. All units are staffed by surgical schedulers, patient service representatives, nurses, and GI technicians. Anesthesiology is present for all GI procedures. Nurses rotate between the admission (Pre op), procedure suites, recovery (PACU) and pre-admission testing. They must be familiar with all roles and procedures in the GI Unit. Job Description The PICU is seeking a Registered Nurse to join our team in Wilmington, DE.This is a 36-hr/wk nightshift position. The Pediatric Intensive Care Unit of Nemours Children's Health, Delaware is a 24-bed unit in a Level 1 Trauma Center. Patients requiring care and surveillance in the PICU will have conditions and diagnoses which may include, but are not limited to, those producing: acute and reversible loss of consciousness, respiratory failure, compromised air exchange, cardiovascular dysfunction, hepatic failure, hemodynamic instability, renal dysfunction, central nervous system injury or dysfunction, acute trauma and organ failure requiring transplantation. Treatments or interventions performed in the PICU include measures utilizing advanced life support technology. These include, but are not limited to cardiopulmonary resuscitation, tracheal intubation, ECMO, pharmacologic support of circulation, mechanical ventilation, point of care monitoring, CRRT, thoracentesis, and hemodynamic, intracranial pressure, intravascular monitoring. Each patient room has electrocardiographic, pulse oximetry, respiratory, hemodynamic and intracranial pressure monitoring capability, as well as central EKG and respiratory monitoring. Portable monitoring during patient transport is provided. Special equipment includes radiant warmer beds, heating/cooling blanket systems, Mapleson manual ventilation system, end tidal CO2 monitors, ventilators, hemodynamic monitoring systems, intracranial pressure monitoring system, ventriculostomy systems, infusion pumps, blood warmer, continuous renal replacement systems, and peripheral nerve stimulators. Requirements Be a graduate of an accredited school of nursing; BSN preferred. Experienced pediatric intensive care nurse of at least 2 years preferred. Current Delaware or Multistate Compact License Must be CPR certified by the American Heart Association. PALS and ACLS preferred.
