Demonstrates ability to adjust to changes in units/shift assignments to meet resident and facility needs. SUMMARY OF QUALIFICATIONS 1. Current license as a Registered Nurse in the State of New York required. 2. Is a graduate of an accredited school of Registered Nursing. 3. Successfully completes facility conducted orientation, mandatory training and inservice programs. 4. Previous long term care experience is required. Supervisory experience and organizational skills in a long term care setting preferred. 5. Must meet health assessment requirements, including two stage Mantoux skin test. 6. Is listed in good standing as confirmed on the New York State Department of Professional Credentialing. 7. Possesses excellent technical assessment and documentation skills, and leadership quantities. 8. Must be capable of performing the essential functions of the job, with or without reasonable accommodations. 9. Must be able to communicate in English, both verbally and in writing, and possess sufficient communication skills to perform the tasks required. 10. Successful completion of facility conducted orientation on specialized dementia units, as appropriate. As a Registered Nurse 2 (Community), you would directly provide, oversee, and coordinate nursing and health related care services for a caseload of individuals in a community setting, such as a Family Care Program, Mobile Integration Team, and outpatient clinic; perform clinical case management activities; serve as a member of an interdisciplinary treatment team; and provide nursing-specific clinical, consultative, and instructional assistance to individuals and their families, other disciplines, and health care providers.
