Works with participating sites to develop mechanisms for the sites to share the patient data (for those requiring follow-up) in a manner that utilizes their existing data and optimizes their time and effort. Completes all necessary electronic records and paperwork regarding patient interactions and appointments. Develops and maintains relationships with community leaders, community-based organizations, faith-based organizations, health institutions, schools, daycares, etc. Attends community-based events, including health fairs, support group meetings etc. Assists with the planning and execution of forums, special events and presentations to reach and educate the community. Supports reporting and paperwork to support funding and grant requirements, including tracking patient metrics such as appointment no-shows and lost to follow up ratios. Tracks all outreach and navigation activities and provides monthly reports. Demonstrates an awareness of self-responsibility and accountability for own professional practice and development.
