A Brief Overview The Care Transitions Coordinator is responsible for facilitating and coordinating services for patients through the continuum of care. The Care Transitions Coordinator ensures appropriate utilization of resources and safe delivery of care to all patients and provides quality and cost-effective care that will lower Length of Stay (LOS) and decrease the fragmentation of services that can potentially compromise patient care. Care Transitions Coordinator collaborates efforts as a team in order to perform one or more of the following roles: Discharge Planning and Disease Management of acute and chronic conditions, facility placement needs as needed and timely referrals and evaluations of all post-acute care needs. The Care Transitions Coordinator play a pivotal role for patients as part of the interdisciplinary team in order to ensure patient safety, quality of care and positive patient outcomes. Rotation of coverage for weekends and holidays will be required and are assigned throughout the year. What You Will Do Modification of plan of care, as necessary, to meet the ongoing needs of the patient Communicate relevant information to third party payors and the care team. Completion of all required documentation in patient records Issues Notices of Non-Coverage per hospital policy. Refers appropriate cases for social work intervention based on department criteria Knowledgeable about Indigent Medication Programs Verifies patient demographics and insurance information is correct Expedites execution of plan of care for patients in Observation units. Arranges transportation for patients slated for discharge. Uphold compliance of regulatory standards for Observation patients including CMS requirement for Code 44. Utilizes “Patient Choice” list for support services post discharge and documents. Initiates home care referrals for VNS, orders durable medical equipment and arranges ambulance transportation as needed for patients prior to discharge or transfer to another facility. Participates as an active, informative and cooperative team member of the health care staff by attending discharge rounds for case finding/follow up and participating in the discharge plan while acting as an advocate for the patient. A Brief Overview The Care Transitions Coordinator is responsible for facilitating and coordinating services for patients through the continuum of care. A Brief Overview Assists in the coordination of the clinical and leadership activities of the Nursing Unit. Collaborates with the Unit Directors in developing, assessing, planning, implementing, directing, and evaluating patient care. What You Will Do Identifies staff educational needs and functions as a clinical resource/role model for nursing personnel Coordinates orientation of nursing employees on specific shift Oversee unit-based preceptor program Participates in the interviewing and hiring of new staff members Participates and involves nursing staff in evaluative nursing research and or performance improvement activities Utilizes evidence-based practice and current research in the development of procedures and protocols Provides for a positive environment for nursing staff Demonstrates knowledge of conflict resolution Assists Nurse Manager/Director with criteria based performance evaluations and competency assessments Addresses shift-specific matters via staff meetings to further enhance communication and identify areas of concern Demonstrates effective communication with other departments Responds to identified concerns of patients, physicians, and visitors. Overview Conducts and reviews UAS-NY comprehensive assessments of potential and existing members. Establishes an individual Plan Of Care based on members clinical, environmental, and social needs to support member safety in the community. Identifies solutions that promote high quality and cost-effective health care services in accordance with VNS Health Plans policies, state and federal regulations.. Manages requests for services from providers, members, and care management team. Works under general supervision. • Conduct face-to-face or telehealth UAS-NY assessments according to state guidelines, policies, procedures, and protocols. • Utilize clinical skills to assess and document all aspects of the potential members long-term community-based needs. • Communicate with members, families, providers, and other parties as needed to complete an accurate comprehensive assessment. • Utilizes VNS Health and state-approved assessment questionnaire, guidelines, and documentation as well as interviews with members, family, and care providers in decision-making. • Performs in-home assessment for members who have identified significant changes in condition since last in-home assessment; provides comprehensive review and determination of member’s needs, including completion of UAS assessment questionnaire, tasking tool, and a projected service plan. Visits include all areas serviced by VNS Health Plans including upstate and downstate counties. • Performs in-home assessment on members to determine the appropriate service plan, including completion of UAS assessment questionnaire, tasking tool, and a projected service plan. Visits include all areas serviced by VNS Health Plans. • Explains VNS Health Plan benefits, including an explanation of the member's handbook. • Ensures compliance with state and federal regulatory standards and VNS Health Plans policies and procedures. • Identifies opportunities for alternative care options and contributes to the development of a safe member centered service plan. • Consult with supervisor and others in overcoming barriers in meeting goals and objectives. • Maintains current knowledge of organizational and state-wide regulations that affect member eligibility. • Coordinates with other departments, e.g. Care Management, Legal Affairs, Grievance and Appeals, Compliance, Membership Eligibility Unit, Quality as needed. • Participates in requests for out-of-network services when a member receives services outside of VNS Health Plans network services. • Keeps current with all health plan changes and updates through on-going training, coaching and educational materials. • Participates in special projects and performs other duties as assigned.
