Description The (Per Diem) Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care 1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. 2. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. 3. Integrates relevant assessment and intervention skills in the delivery of nursing care. 4. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. 5. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care and delegating to others as appropriate. 6. Educates patients and caregivers while anticipating needs and readiness to learn, about their plan of care, transition of care, promotion of health, and prevention of disease. 7. Communicates effectively and professionally with patients, family and all members of the Interdisciplinary Patient Care Team. 8. Manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates allocation of material resources effectively. Patient Experience 1. Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions including, but not limited to, Admission Welcome, Bedside shift Report, HELP, AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. 2. Quality and Safety 1. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. 2. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. 3. Implements process improvement strategies based on institutional, evidence-based ensure and procedures. 4. Engages in formal and informal peer and institutional review processes. 5. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies. 6. Contributes to quality and safety practices and required compliance measures. 7. Implements principles of high reliability to identify and avoid high risk behaviors to provide a safe environment for patients. 8. Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol. 9. Utilizes institutional information technology by documenting nursing practice to support quality and performance improvement initiatives. Operations 1. Performs charge nurse duties as assigned. 2. Coordinates with the Clinical Coordinator/Clinical Nurse Manager/Assistant Nurse Manager/Nursing Shift Manager to ensure supplies are appropriately utilized and inventory is sufficient for patient care. 3. Escalates material or staffing resource deficits to charge nurse, Clinical Nurse Manager/Assistant Nurse Manager or Nursing Shift Manager. 4. Demonstrates patient focused and cost effective approaches to patient care in terms of equipment, staff, supplies and all other resources. 5. Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care. Professional Development 1. Advances clinical competence in nursing practice to progress from novice to expert. 3. (BSN) Associate’s Degree in Nursing (ADN) RN who is matriculated in a BSN Program with a graduation date within the year. The Prep & Recovery Unit RN cares for patients who will be receiving anesthetic care and who are recovering from anesthesia after surgery. They monitor pre and post-operation vital signs, assess levels of consciousness, and carefully observe patients for any side effects from anesthesia. Roles and Responsibilities: Performs and documents an accurate assessment on each assigned patient within established standard of care. Documents flow sheets, graphic sheets, and anesthesia signature are completed by end of recovery phase and reflects patients progress toward established goals. Ability to appropriately problem solve incidents on the unit. Communicates incident information and action taken to the appropriate people. 4 - 10 or 3 -12 hour variable shifts. Unit operating our hours 5a-10p. Shifts will flex between pre-op and post-op and will be based on surgical schedule. Call component is included in this role. One weekend 24 hour call shift in a 6 week rotation. Weekday call begins at 9p-7am. Some holiday call. Required Skills and Experience: Graduate of an accredited nursing program New York State Registered Nurse licensure Preferred Skills and Experience: BSN 1-3 years of OR experience ACLS, PALS Physical Requirements: Able to lift 20 pounds. The Chess Team registered Nurse will provide nursing care to a wide variety of patients within the Cayuga Health System. During every shift, the Chess Registered Nurse will be member of a nursing team that provides direct nursing care to a specific unit within the system. It is conceivable that a Chess Registered Nurse may start their shift in one unit and then based on a change in census and acuity be transferred or moved to a different unit to meet operational requirements. While working within a nursing unit the Chess Registered Nurse acts as a patient advocate and facilitates communication between the patient, family, and other healthcare professionals. This results I a holistic approach to planning, organizing, teaching and relationship development that best meets the needs of the patient. Roles and Responsibilities: Assess, diagnose, plan, implement, and evaluate nursing plan considering the patient and family's individual, cultural, ethnic, and religious values and beliefs Provide patient education for diagnostic procedures, medical conditions and treatment plans Provide nursing care utilizing defined competencies and skills to areas assigned. Demonstrates an aptitude and willingness to learn new responsibilities Excellent organization, interpersonal, and customer service skills Ability to work independently, make decisions, meet deadlines, multi-task and solve problems Will be designated to train and work in various areas of facility providing safe & therapeutic patient care Requires excellent communication skills, pleasant demeanor and positive attitude Able to work well with others in a teamwork environment. Required Skills and Experience: Associates of Applied Science in Nursing Current NYS RN license BLS for all units and some may require ACLS and/or PALS Preferred Skills and Experience: Previous critical care experience Physical Requirements: Repetitive use of hands and fingers (g. use of the computer keyboard) light lifting in regards to patient care. Ability to sit, stand, or walk for long periods of time as it pertains to patient care. Must be physically able to operate the equipment used for the job.
