Duties Description The candidate will clinically oversee a cluster of nurses and be responsible for multiple sites and may function as a RN1 as needed. Ensure nursing case management of multiple locations; caseload based on medical acuity. Oversee and supervise RNs and LPNs who will supervise the AMAP staff in providing Direct Support Assistants health care per the nurse Act Exemption. Ensure all medical follow-up is done timely. Responsible for in-service on nursing plans and other staff development and oversight as required, including medication administration, which are vital activities to regular operations. Engage in orientation and mentoring of new RNs and LPNs. Complete evaluations. Monitor/Complete nursing audit for the cluster, work closely with the Nursing Supervisor / Director of Nursing and Team to review findings, and develop a plan of correction. Monitors individuals’ medical/health needs and services. Interacts with primary medical specialists, hospitals, and various clinical staff as necessary. Advocate for individuals’ health care needs. Maintains ongoing communication with med liaison/designee. Provides support to site staff in maintaining health services for the individuals. Utilizes appropriate reporting mechanisms/chain of command in responding to individual and/or staff issues. Responds to emergency requests as soon as possible (within and outside caseload). Completes nursing assessments and related documentation via elect, e.g., RN Notes, nursing logbook, etc., thoroughly and within established time frames. Provides individual-specific nursing interventions. Responds to staff questions regarding individual-specific medical/nursing issues. PONS is current (for caseload) and signed by the staff. All PONS are maintained in EHR. Provides health-related training/consultation to residential staff. Annually, for each person in your caseload, complete: a review of the medical section Life Plan/CFA, medication audit (each home/day program). Participates in Nurse on call rotation. Obtain individual’s informed consent for medications, as per BDDSO Policy. Completes required medication error documentation and staff medication error remediation. Maintains professional skills. As we continue to grow, we are looking for a motivated and organized RN Assistant to work alongside our Director of Nursing (DON) and play an essential role in our clinical operations. What You’ll Do As an RN Assistant, you’ll help keep our clinical department running smoothly by supporting both administrative and client-related tasks: Partner closely with the Director of Nursing on daily operations Organize and maintain client records and documentation Conduct 30-day follow-up calls with clients Assist with (or be trained to perform) 60-day client visits Support caregiver communication by providing client information to CHHAs Help with scheduling, documentation, and office coordination Maintain accurate logs and ensure compliance with agency standards Deliver excellent customer service to clients and families What We’re Looking For We’re seeking someone who is dependable, detail-oriented, and compassionate: Experience in homecare or a healthcare office (preferred, not required) Strong organizational and multitasking skills Comfortable speaking with clients over the phone and in person Self-motivated with the ability to work independently and in a team Professional, warm, and client-focused attitude Basic computer skills (EMR, scanning, filing—training provided) Availability for at least 3 full days per week Why Work with Us We value our team and offer a supportive environment where you can grow: Competitive pay Profit sharing opportunities 401(k) with employer match Paid sick time Career growth and advancement opportunities Friendly, team-oriented workplace Schedule Part-time or full-time available Minimum requirement: 3 full days per week How to Apply We’d love to hear from you!
