Position in this function is responsible for the effective delivery and management of direct patient care utilizing the nursing process and adhering to standards of nursing practice. Provides appropriate care to patients ranging within the ages of infants to geriatrics. Provides pre-visit and post-visit telephonic support and patient education, populating the Electronic Medical Record with historical information, lab findings, and preliminary orders to be reviewed by the physician at the next clinic visit. Ad hoc visits to the clinical sites will also be scheduled to enhance coordination of team-based care. (45%) Queries providers to clarify missing, ambiguous, or conflicting documentation in compliance with ACDIS/AHIMA query guidelines. (15%) Collaborates with providers and healthcare teams to ensure documentation supports risk adjustment and quality measures. (10%) Educates providers on best documentation practices, HCC coding, and medical necessity, to include content creation and delivery via one-on-one and group sessions. Educate patients about their treatments and address any concerns about adverse reactions. Administer prescribed medications and establish IV access, including central lines. Monitor and document patients' conditions and responses to treatments. Ensure accurate and timely EMR documentation. Maintain the flow of patient care and manage emergency situations calmly and effectively. Keep infusion rooms clean and well-stocked.
