Job Summary Enliven is seeking an experienced RN (Registered Nurse) to service home infusion and Infusion Center patients in Spring and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will educate patients and their families on prevention and healthy habits. The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote patients' well-being by providing high-quality nursing care. Responsibilities: RN (Registered Nurse) Complete daily electronic forms for each assigned patient within a secure Electronic Medical Record (EMR) System. Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients. Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager. Confer with physicians and pharmacies to develop the initial treatment plan based on the physician's orders and the initial patient assessment. Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation. Report patient care, conditions, and progress to the patient's physician, pharmacy, and Clinical Manager continuously. Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems. Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation. Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimal patient care. Duties & Responsibilities RESOURCE MANAGEMENT Ensures operation of equipment by completing preventive maintenance requirements; following manufacturer's instructions; troubleshooting malfunctions; calling for repairs; maintaining equipment inventories; evaluating new equipment and techniques. Appropriately utilizes all nursing supplies and equipment. Demonstrates effective teamwork skills and maintains a cooperative relationship among health care teams by communicating information; responding to requests; building rapport; participating in team problem-solving methods. Promptly responds to all communication devices in order to meet patient needs. Assists in performing and documenting environmental checks as required by LMC policy and regulatory agencies. PROFESSIONAL RESPONSIBILITIES Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies. Meets or exceeds all LMC requirements for attendance and service expectations. Maintains required certifications. Completes yearly safety training and other mandatory classes. Ensure Hazardous Waste or Hazardous Waste Pharmaceuticals are discarded, stored, and managed with other compatible waste streams in containers that are properly labeled and closed prior to being shipped offsite for proper disposal. Initial RCRA training is received within 6 months of the hire date and continuing with annual RCRA training. Earns a minimum of 15 hours of continuing education annually within specialty.
