The selected caregiver must be available to work all three scheduled days Monday, Wednesday, and Friday from 12:00 PM to 5:30 PM. Consistent attendance is essential because the client depends on a stable routine and familiar caregivers. The client also has a small dog in the home. Meal Preparation, Snacks, and Hydration Depending on the client’s care plan and preferences, responsibilities may include: Preparing simple meals and snacks Encouraging appropriate food and fluid intake Providing hydration reminders Assisting with meals when required Cleaning food-preparation areas after use Washing dishes and returning items to their proper locations Observing and reporting appetite changes, swallowing concerns, nausea, or refusal of food or fluids Following all dietary instructions listed in the client’s care plan Attention to cleanliness, nutrition, hydration, and the client’s comfort is essential. Light Housekeeping and Home Support The caregiver will help maintain a clean, organized, comfortable, and hazard-free living environment. Responsibilities may include: Tidying commonly used areas Washing and putting away dishes Completing light laundry Changing bed linens when needed Wiping down frequently used surfaces Keeping the bathroom and personal-care areas clean Removing trash Returning supplies and mobility equipment to their proper locations Keeping floors and walking paths free from fall hazards Reporting low supplies, maintenance concerns, or household safety hazards The client’s direct care, supervision, and safety must always remain the caregiver’s first priority. Observe, Document, and Report Changes The caregiver must closely observe the client’s overall condition and promptly report concerns to Helping Hearts. This includes changes involving: Mobility, balance, or transfer ability Physical strength or coordination Skin condition, bruising, redness, swelling, or pain Appetite or fluid intake Toileting patterns Mood, behavior, or cognition Alertness, confusion, agitation, or unusual behavior Breathing, comfort, or general condition The safety or cleanliness of the home Supplies that are low or unavailable Any fall, near fall, injury, or other incident Caregivers must complete accurate shift documentation, electronic visit verification, and required care notes through the company’s approved system. Concerns must be reported immediately. Caregivers must not wait until the end of the shift to report an urgent change, fall, near fall, injury, or safety concern. Small Dog in the Home The client has a small dog. Applicants must: Be comfortable entering and working in a home with a small dog Remain aware of the dog’s location when the client is walking or transferring Help prevent the dog, toys, food bowls, or leashes from becoming fall hazards Follow the client’s instructions regarding appropriate interaction with the dog Never allow the pet to interfere with the client’s care, supervision, or safety Routine pet care is not automatically part of the assignment unless it is specifically included in the client’s approved care plan. What We Are Looking For Exceptional Reliability and Continuity of Care This assignment requires a caregiver who can consistently work: Every Monday, Wednesday, and Friday from 12:00 PM to 5:30 PM. Frequent call-offs, late arrivals, early departures, or ongoing schedule changes would disrupt the client’s care, safety, and established routine. Job Summary: Works collaboratively with an MD to coordinate and screen for the appropriateness of admissions and Continued stays. Makes recommendations to the physicians for alternate levels of care when the patient does not meet the medical necessity for Inpatient hospitalization. Interacts with the family, patient and other disciplines to coordinate a safe and acceptable discharge plan. Functions as an indirect caregiver, patient advocate and manages patients in the most cost effective way without compromising quality. Transfers stable non-members to planned Health care facilities. Responsible for complying with AB 1203, Post Stabilization notification. Complies with other duties as described. Must be able to work collaboratively with the Multidisciplinary team, multitask and in a fast pace environment. Essential Responsibilities: Plans, develops, assesses and evaluates care provided to members. Collaborates with physicians, other members of the multidisciplinary health care team and patient/family in the development, implementation and documentation of appropriate, individualized plans of care to ensure continuity, quality and appropriate resource use. Recommends alternative levels of care and ensures compliance with federal, state and local requirements. Assesses high risk patients in need of post-hospital care planning. Develops and coordinates the implementation of a discharge plan to meet patients identified needs. Communicates the plan to physicians, patient, family/caregivers, staff and appropriate community agencies. Reviews, monitors, evaluates and coordinates the patients hospital stay to assure that all appropriate and essential services are delivered timely and efficiently. Coordinates the interdisciplinary approach to providing continuity of care, including Utilization management, Transfer coordination, Discharge planning, and obtaining all authorizations/approvals as needed for outside services for patients/families. Acts as a liaison between in-patient facility and referral facilities/agencies and provides case management to patients referred. Refers patients to community resources to meet post hospital needs. Coordinates transfer of patients to appropriate facilities; maintains and provides required documentation. Adheres to internal and external regulatory and accreditation requirements and compliance guidelines including but not limited to: TJC, DHS, HCFA, CMS, DMHC, NCQA and DOL. Educates members of the healthcare team concerning their roles and responsibilities in the discharge planning process and appropriate use of resources. Provides patients with education to assist with their discharge and help them cope with psychological problems related to acute and chronic illness. Per established protocols, reports any incidence of unusual occurrences related to quality, risk and/or patient safety which are identified during case review or other activities. Reviews, analyses and identifies utilization patterns and trends, problems or inappropriate utilization of resources and participates in the collection and analysis of data for special studies, projects, planning, or for routine utilization monitoring activities.
