Description Of Position The LPN is responsible for providing comprehensive nursing care to patients and their families under the direction of the registered nurse. The position will perform duties within the scope of practice, ensuring consistency with LCMH policies and procedures, standards of care, approved protocols and provider orders. The position will also adhere to practice standards governed by the Louisiana State Board of Nursing. SUPERVISION: Reports directly to Nurse Manager and/or Charge Nurse Responsibilities And Duties Performs assessments and reassessments of patients. Documents accurately according to hospital policy. Possesses competency in use of technology and computers. Demonstrates competency in the use of smart pumps. Demonstrates competency in medication administration. Organizes patient care and delegates to unlicensed staff as appropriate. Collaborates with interdisciplinary team in planning and organizing patient care from admit to discharge. Demonstrates the following best practices in a consistent and positive manner: Huddles, Service Excellence principles, and competency in unit based procedures. Follows evidence-based policies and procedures for nursing practice. Competently performs unit specific procedures. Assist the RN in development and implementation of plan of care. Encourages and supports non licensed personnel. Consistently participates in the multidisciplinary approach to patient care. Makes suggestions/recommendations for improved operations. Has knowledge of and utilizes unit specific equipment appropriately. Follows established hospital/department safety policies. Understands and appropriately responds to the unique combination of cultural variables that professionals and individuals bring to interactions. Follows hospital/department infection control policies. Is knowledgeable of and actively participates in implementation of the Unit PI plan. About this Job: General Summary of Position The Medical Assistant performs clinical and administrative duties in an ambulatory care setting under the supervision of a physician/provider and/or a registered nurse. Duties Description POSITION DESCRIPTION: Registered Nurses 2 (Community) are registered professional nurses who provide, oversee, coordinate, and evaluate nursing and health related care and treatment services for individuals* in community settings. Provides instruction to individuals regarding self-care, self-medication, symptom management, and preventive health strategies. Instructs family members and employees/providers on how to appropriately respond to mental health crises or medical emergencies. Intervenes in crisis situations and administers emergency treatment. Maintains current knowledge and training in cardiopulmonary resuscitation (CPR) and infection control; and instructs individuals and staff on precaution and prevention strategies. The Nurse Practitioner provides medical and/or psychiatric evaluation, assessment, medication management, care coordination, and ongoing treatment services for individuals receiving behavioral health and substance use disorder treatment services. This role collaborates with interdisciplinary treatment teams and internal and external healthcare providers to support high-quality, person-centered care in accordance with organizational, regulatory, and professional practice standards. Address: 420 Supreme Ct, East Stroudsburg, PA 18302 Shift: (In-Person) Sunday, Monday, Tuesday, and every other Wednesday - 12 hour shifts - 7:00am - 7:00pm or 8:00am - 8:00pm - On-Call via phone or virtually Pay: (The pay range depends on relevant experience, is a plus /FNP/CRNP) Essential Duties and Responsibilities: Complete history and physical assessments, evaluations, reassessments, and other clinical documentation as required. Review laboratory results, monitor blood levels, review physician orders, and support ongoing medical and medication management needs. Provide medication management services, including psychotropic medication management where applicable based on licensure and scope of practice. Diagnose and treat medical and/or psychiatric conditions in accordance with scope of practice and applicable regulations. Coordinate client care with internal and external healthcare providers, including primary care physicians, psychiatrists, specialists, pharmacies, and other treatment providers as appropriate. Support discharge planning and continuity of care efforts, including medication coordination and aftercare recommendations. Collaborate with nursing and clinical teams to support cost-effective medication management and completion of medication prior authorizations when needed. Provide education and clinical support to staff regarding medical, psychiatric, behavioral health, and medication-related concerns as appropriate. Participate in treatment team meetings, clinical review meetings, peer review processes, and departmental meetings as required. Maintain timely, accurate, and compliant documentation in accordance with CARF standards, licensing requirements, payer requirements, and organizational policies. Must exercise discretion and maintain confidentiality with regard to all company information. Completion of all required trainings as designated by the company and a ccreditation/licensing entities.
