In this work-from-home role, you’ll deliver after-hours care virtually (primarily by phone) to aging residents across multiple states. This position offers both autonomy and purpose—you’ll be the trusted voice and clinical partner helping residents and facility staff during critical times, making an immediate impact in the lives of older adults. In this position the provider must be comfortable managing high call volumes and performing at least 30% telehealth visits, including evaluation of acute changes, falls, and controlled substance visits. Providers must be able to manage multiple calls independently while providing care across several states. Essential Duties & Responsibilities Serve as the first line of support for residents and facility staff after-hours, providing direction and medical care over the phone. Use Curana’s telephonic platform to take and place calls, coordinating care between facilities, hospitals, and clinics. Deliver high-quality, cost-effective care to patients—addressing acute, chronic, and behavioral health needs in collaboration with physicians and specialty providers. Perform comprehensive assessments and document encounters accurately and thoroughly in the EMR, ensuring compliance with CMS requirements. Apply Curana’s clinical protocols and practice guidelines to support safe, effective treatment in place whenever possible. Participate in mandatory education and training to stay current with standards of care. Scheduling & Hours: While shift times can vary, we provide coverage to skilled nursing and senior living facilities on weeknights from 5pm- 8am local time, continuous coverage from Friday at 5pm to Monday at 8am. Holiday coverage is also provided beginning at 5pm of the end of the last business day to 8am of the resumption of business hours. Assumes ultimate responsibility for all resident care delivered on the shift by assigning staff including: a. assessing, evaluating, and following-up on the delivery of quality resident care according to facility and CF Operating Policies/ Procedures/Guidelines/Playbooks/Manuals b. reviewing staff documentation regarding resident care to assure timeliness and accuracy. c. completing periodic rounds on wards/pods with assessment, evaluation of quality nursing and direct care. d. monitor resident care through observation and communication with staff supervised. e. ensuring adequate supplies and equipment are available for resident care. f. assisting in handling medical/psychiatric emergencies with accurate documentation. g. 1-on-1 observation of nurses while administering medication/treatments at least once a year or more often as needed. h. assigning and monitoring medication education groups for specific needs. Providing medical emergency and emergency pregnancy review/drills on shift assigned. c. Providing immediate intervention to de-escalate a resident to help decrease potential for self-harm or harm to others and potential need for seclusion/restraint process. d. Reading Personal Safety Plans on all residents assigned to be familiar with their personal preferences when they are upset to help de-escalate their behaviors and reduce potential for S/R episodes. e. Documenting all pertinent information during a manual, seclusion/restraint process with proper forms completed within timeframes and follow-up documentation in chart as well as ways to potentially decrease incidents in the future. Serves as resource person to other stafff or training or continuing education programs. Participates in attendance to in-house and outside training activities (Train the Trainer, etc.) for updating staff on skills and knowledge. Other related duties as required. OVERVIEW: Responsible for the overall operation and optimal quality of care for the residents on the assigned unit. The Emergency Department Registered Nurse performs assessments appropriate to the patient’s clinical presentation/diagnosis, care setting, age, development level, and cultural and religious orientation in a timely manner. Performs assessments completely and accurately including, but not limited to, physical (i.e. assessment for pain, skin breakdown, risk for falls), psychological (i.e. cognitive and communicative skills or development), social status and other patient needs. Qualifications Essential Job Duties : Identifies and prioritizes the need for further assessment based on the patient’s clinical presentation/diagnosis, care setting, desire for care and responses to any previous care. Initiates discharge planning for each patient upon admission as appropriate. Is able to monitor hemodynamic status of patient and correctly interpret the results. Demonstrates knowledge of cardiac monitoring and identifies dysrhythmias and treats appropriately. The ER RN must be able to accurately assign triage categories using Emergency Severity Index. Is able to assist emergency room physician with diagnosis and interventional procedures, i.e., central lines, closed reductions, advanced airway management. Is competent to monitor the patient undergoing procedural sedation. Is competent to perform ACLS and PALS algorithms and protocols.
