General Purpose The primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. Essential Duties • Use the wristband or photo card file to identify residents before administering treatments, serving meals, etc., as necessary. • Use only authorized abbreviations established by this facility when recording information. • Report all changes in the resident’s condition to the Nurse Supervisor/Charge Nurse as soon as practical. • Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner. • Report all accidents and incidents you observe on the shift that they occur. • Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the Administrator. • Report any known or suspected unauthorized attempt to access facility’s information system. • Perform only those nursing care procedures that you have been trained to do. • Ensure that the resident’s room is ready for receiving the resident (i.e., bed made, name tags up, admission kit available, etc.). • Greet residents and escort them to their room. • Introduce resident to his/her roommate, if any, and other residents and personnel as appropriate. • Make resident comfortable. • Inventory and mark the resident’s personal possessions as instructed. • Store resident’s clothing. • Assist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged. • Transport residents to new rooms or to the receiving area. • Assist with loading/unloading residents’ to/from vehicles as necessary. • Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift. • Meet with your shift’s nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services. • Report all complaints and grievances made by the resident. • Participate in and receive the nursing report as instructed. • Follow established policies concerning exposure to blood/body fluids. • Make beds as instructed. • Put extra covers on beds as requested. • Measure and record temperatures, pulses, and respirations (TPRs) of residents, as instructed. Weigh and measure residents as instructed. • Ensure that residents who are unable to call for help are checked frequently. • Answer resident calls promptly. • Check residents routinely to ensure that their personal care needs are being met. • Assist residents with identifying food arrangements (i.e., informing resident with sight problem of foods that are on his/her tray, where it is located, if it is hot/cold, etc.). • Record the resident’s food/fluid intake. Report changes in the resident’s eating habits to the Nurse Supervisor/Charge Nurse. • Keep residents’ water pitchers clean and filled with fresh water (on each shift), and within easy reach of the resident. • Perform after meal care (i.e., remove trays, clean resident’s hands, face, clothing, etc.). • Participate in and receive the nursing report upon reporting for duty. • Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors. • Follow work assignments, and/or work schedules in completing and performing your assigned tasks. • Cooperate with inter-departmental personnel, as well as other facility personnel to ensure that nursing services can be adequately maintained to meet the needs of the residents. • Notify the facility when you will be late or absent from work. • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents. • Assist residents with daily functions (dental and mouth care, bath functions, combing of hair, dressing and undressing as necessary). • Keep residents dry (change gown, clothing and linens, when it becomes wet or soiled). • Assist residents in preparing for medical tests (i.e., lab work, x-ray, therapy, dental, etc.). • Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. • Perform restorative and rehabilitative procedures as instructed. • Assist in preparing the resident for a physical examination. • Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes. • Observe and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores). • Report injuries of an unknown source, including skin tears. • Observe disoriented and comatose residents. Record and report data as instructed • Perform special treatments as instructed. • Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving/missing from the facility. • Use only the equipment you have been trained to use. • Report defective equipment to the Nurse Supervisor/Charge Nurse • Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information. • Report all allegations of resident abuse and/or misappropriation of resident property. • Honor the resident’s refusal of treatment request. Report such requests to your supervisor. Supervisory Requirements This position has no supervisor responsibilities Qualification Education and/or Experience Must possess, as a minimum, an 10th grade education Language Skills Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from managers and employees. Mathematical Skills Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations. Reasoning Ability Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Certificates, Licenses, Registrations Must be a licensed Certified Nursing Assistant in accordance with laws of the state. Assures that all supplies utilized for TA procedures/treatments comply with required standards for inventory and quality control. Maintains Therapeutic Apheresis Preparation Packets, assuring that they have the most current forms & process checklist; and are available for physician access on 4W and PICU for emergency procedures. Maintains logs and reports TA quality control issues-- errors, variances, incidents and complaints—to Stem Cell Transplantation Program (SCTP) Quality & Safety Committee. Reviews and revises TA Standards Of Practice (SOP) annually under the direct supervision of the Therapeutic Apheresis Medical Director; assures compliance with current accreditation and regulatory agency standards for TA. Responsible for TA document control and distribution of all TA-related documents. Collaborates with Hematology, Oncology, & Transplantation Clinical Educator and Leadership to assure appropriate orientation for all new TA nurses. Summary The Division of Gastroenterology, Hepatology and Nutrition (GHN) at Texas Children's Hospital (TCH) is developing a team of close-knit Baylor College of Medicine (BCM) Physician Assistants and Nurse Practitioners at the Texas Medical Center campus to be part of a dynamic and collaborative multidisciplinary medical team who will care for hospitalized patients with a variety of gastrointestinal and liver disorders. This position offers a unique experience on our in-patient service lines for an individual who wishes to pursue a career in pediatric gastroenterology, hepatology and nutrition, build relationships with patients/families and be immersed in a highly educational environment with frequent opportunities to attend research lectures, pathology conference, and obtain CME through GI grand rounds and fellows workshops. The Division of GHN at TCH is currently ranked the #5 program in the country by US News and World Report and its faculty are internationally recognized clinical experts and researchers. As the largest children's hospital in the country, TCH is a major quaternary referral center for some of the rarest GI and liver disorders, allowing for unparalleled development of pediatric GI and hepatology expertise. Currently, three gastroenterology, hepatology, and nutrition in-patient service lines care for 20 - 35 patients per day with a wide range of gastrointestinal, liver, and nutritional disorders including but not limited to: inflammatory bowel disease, short bowel syndrome/ intestinal failure, gastrointestinal motility disorders, liver diseases, liver transplantation, and pancreas and bile duct abnormalities. Management of nutritional support in the form of enteral tube feeding and parenteral nutrition is also provided. The service line also performs a variety of diagnostic and therapeutic endoscopic procedures. The physician assistant/ instructor will work alongside faculty colleagues, pediatric gastroenterology and transplant hepatology fellows, pharmacists, dieticians, social workers and pediatric residents to care for this pediatric patient population in both the acute care and ICU settings. We seek individuals who are organized and detail-oriented and can work well with staff and faculty from different departments to coordinate outstanding patient care. Essential Job Responsibilities: The RN provides care to patients who have been diagnosed with a terminal illness. The Hospice RN regularly reviews and updates the plan of care. Completes an initial, comprehensive, and ongoing assessments of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. Acts as Case Manager when assigned by Clinical Supervisor/Nursing Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Completes, maintains, and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. Communicates with community health related persons to coordinate the care plan. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.
