Update patient demographic information and coordinate follow-up appointments. Assist with obtaining and organizing clinical documentation for provider review. Request medical records from outside facilities and scan appropriate documentation into the EMR. Maintain patient privacy and confidentiality in accordance with HIPAA requirements. Clinic & Team Support Provide professional, courteous and compassionate service to patients, caregivers and team members. Communicate clinic needs, concerns and activities to leadership. Assist with telephone calls and route inquiries to the appropriate team member. Follow established safety and infection-prevention practices. Job Description Summary Ashley River Tower (ART) perioperative services is located on the fourth floor of the ART building. The ART operating room offers adult outpatient and inpatient surgeries through various surgical service lines including general, plastics, oncology, cardiac, thoracic, heart and lung transplants, vascular, colorectal, bariatric, and robotics. Operations are Monday through Friday, 7:00am to 7:00pm. General, Vascular, and Cardiothoracic call teams are available to cover emergency cases prior to the posted schedule times and on the weekends. There is 24-hour charge nurse coverage for after hours and weekends. The clinical programs that service ART are recognized for excellence in cardiac surgery, thoracic surgery, vascular surgery, transplant surgery, as well as general and GI surgery and is nationally recognized for management of chronic pancreatitis, and bariatric surgery. The CNA/HHA works with other licensed care staff to coordinate the proper types of care to residents and strictly follows all Company nursing policies and procedures as well as safety guidelines. Role Overview As a Home Health RN, you are the clinical lead for your patients - managing the plan of care, coordinating across disciplines, and driving outcomes from admission through discharge. You’ll combine advanced assessment skills, care planning, and patient education to reduce hospitalizations, improve recovery, and support long-term independence at home. What You’ll Do Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care Develop and manage individualized care plans in collaboration with physicians and care teams Deliver skilled nursing care, including medication management, treatments, and interventions Serve as the case manager, c oordinating care across therapists, aides, and physicians Monitor patient progress and make clinical decisions based on changing conditions Educate patients and families to support disease management and independence Coordinate care transitions and discharge planning to reduce readmissions Maintain timely, accurate documentation in the EMR Supervise and support Home Health Aides as needed Participate in on-call rotation as appropriate What You Bring Active RN license (or compact eligibility) + CPR certification Graduate of an accredited nursing program Minimum one (1) year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred) Strong clinical judgment and ability to think independently Excellent communication skills and patient-centered approach Ability to manage a caseload and prioritize effectively Proficiency with EMR systems Valid driver’s license and reliable transportation What Sets You Apart Experience in home health or post-acute care Experience with case management and interdisciplinary coordination BSN Familiarity with Homecare Homebase (HCHB) What to Expect Field-based role caring for patients in their homes within an assigned territory High level of autonomy supported by a collaborative clinical team A dynamic environment requiring adaptability, critical thinking, and strong time management Performance expectations tied to clinical quality, patient outcomes, and documentation timeliness If you’re looking for a role where you can practice at the top of your license, build real patient relationships, and deliver care that truly changes outcomes - this is it.
