Patient/Family Partnership & Education- Partners with child and family in evaluating outcomes. Monitors, trends, and evaluates patient/family responses to treatment. Provides appropriate education to patients and families regarding all aspects of care according to CCMC policies and standards; supports the organization’s efforts to patient/family education. Triages phone calls utilizing critical thinking and assessment skills to evaluate the chief complaint and urgency of the problem. Triages phone calls to determine acuity to assist in scheduling appointments. Provides comprehensive medical record documentation. Documents all communications and maintains patient confidentiality. Participates in and/or organizes regular practice huddles, communicates effectively with team members and physicians; actively listens and seeks feedback. Collaborates with team members to solve problems and resolve conflicts. Functions as a resource/mentor and provides support to team members to resolve patient care and team concerns/issues. Utilizes standard precautions including, appropriate isolation techniques. Consistently follows the Ambulatory Health Care National Patient Safety Goals.
