Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other specialized tests required for evaluation of illness, and scrutinizes lab data on new and former clinic patients and hospital admissions to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan. 2. Must be able to work with a collaborating physician. 3. Performs and interprets, standard laboratory, radiologic, cardiographic and other routine diagnostic procedures used to identify pathophysiologic processes as privileged by Genesis Credentialing Board 4. Performs routine procedures such as injections, immunizations, suturing and wound care, managing simple conditions produced by infection or trauma, assisting in the management of more complex illness and injury as privileged by the Genesis Credentialing Board. 5. May prescribe and monitor medications as allowed by prescriptive authority through applicable state Board of Nursing/Medicine and Genesis Credentialing Board. 6. May need to take call and work some off-hour shifts as needed. 7. Perform in-hospital consultations. Facilitates appropriate length of stay, discharge planning, and compliance with regulatory standards for inpatient management. 8. Ensures continuity of care by serving as a liaison between patient and other members of the multi-disciplinary care team or with other specialty areas as necessary 9. Participates in quality monitoring thru the review of records and treatment plans for patient outcomes periodically to assure quality care 10. Discharge planning – Dictates discharge summaries, writes prescriptions, explains discharge instructions and confirms follow-up appointments. 11. Other duties as assigned. QUALIFICATIONS 1. APRN: EDUCATION: Graduate of an accredited APRN training program. 2. Current Ohio RN and APRN licensure 3. Must have a current certificate of authority and certificate to prescribe (or eligible) by the Ohio Board of Nursing. 4. Current DEA registration (for Interventional Radiology only) 5. Acute Care, Surgical, Critical Care, or Trauma Experience Preferred. 6. Required certifications: BLS to be obtained within six (6) months of hire. The RNCM maintains accurate and timely records of participant’s functional /health status, progress toward care plan outcomes, revisions to care plans, care given, etc. All charting and documentation is performed in accordance with CSI policies/procedures. The RNCM participates in the collection and documentation of Data PACE information. The RNCM advocates to others on behalf of the participant, and demonstrates accountability in resolving participant concerns or issues. The RNCM understands, complies with and promotes the Participant Bill of Rights and assesses and works toward achieving high levels of participant satisfaction. The RNCM may provide after hours on-call medical assistance on a rotating basis, via phone triage or after hours home visits to participants as needed. Schedule requires a rotating on call shift. KNOWLEDGE, SKILLS AND ABILITIES: Must be a Registered Nurse with current Michigan licensure, BSN preferred. The RNCM participates in annual, mandatory in-service training and screening, including but not limited to: infection control, TB testing, safety training, and BLS training. The RNCM assumes responsibility for self-development through continuing education, utilizing resources within the health care system or elsewhere; the RN promotes professional behavior and growth by serving as a role model within the health team.
