Position Summary: The Registered Nurse (RN) Public Health Case Manager oversees comprehensive case management for individuals requiring public health care, including well-child checkups, immunizations, tuberculosis and other infectious disease screening and diagnostic testing, management of chronic health conditions, and infectious disease case investigations. The Case Manager provides education on topics related to immunizations, chronic care, preventive health examinations, and diagnostic and surveillance testing, and is required to communicate effectively with diverse audiences, ensuring the information presented is understood and applied appropriately. Additionally, the Case Manager assists in providing instruction during Health Aide Training and at Continuing Medical Education (CME) events for CHA/Ps and other clinical staff. Other duties include coordinating with other health care providers, maintaining accurate records of patient interactions, monitoring patient progress, and ensuring patients' health needs are met effectively. The Case Manager must stay updated with the latest public health knowledge and developments, applying this knowledge to improve patient outcomes and overall program effectiveness.
