Prescribe medications, wound therapies, and durable medical equipment within scope and state regulations. Educate patients and caregivers on wound care, chronic disease management, medication adherence, and preventive health. Document promptly and accurately in the EMR, including clear care plans and communication with the broader care team. Participate in case reviews/huddles with the Home Health and medical teams to review panel outcomes and refine protocols. Comfort working independently in the field while collaborating tightly with an interdisciplinary team. Valid driver’s license and reliable transportation for house calls within our service area. How This Role Is Structured You will own a panel of patients attributed to you, with defined visit frequency based on clinical need and program protocols. Mix of home visits and clinic time; precise ratio can be tailored based on patient demand and your strengths.
