Overview The Department of Internal Medicine at Macon & Joan Brock Virginia Health Sciences at Old Dominion University is seeking a Licensed Practical Nurse (LPN) to join the team. Provides nursing care for patients and assisting physicians. Clinical and administrative duties include: Triage patients [vital signs, height/weight, medical/social history, medication reconciliation] Stock/prepare exam rooms Retrieve imaging and laboratory results Process work task/message ques in the electronic health records [EPIC], and the patient portal [myChart] Ability to work in our clinical and administrative areas with the Department of Medicine. Responsibilities Perform patient intake including obtaining vital signs, reconciling medications and allergies, documenting chief complaints, administering medications and immunizations according to approved protocols, observing for adverse reactions, assisting providers with examinations, minor procedures, diagnostic testing, and implementing standing orders. Provide basic patient care and document all nursing interventions in the electronic medical record. Manage patient-related tasks and follow-up activities including responding to patient messages and telephone calls, communicating test and laboratory results, answering patient questions, coordinating referrals and specialist appointments, interacting with family members and healthcare providers, documenting all patient communications, notifying providers of critical results, and ensuring same-day response to patient inquiries when appropriate. Review charts for completeness, monitor test results and task completion, reconcile discharge medications, manage prescription refills, obtain medication prior authorizations, administer PPD testing as needed, and ensure accurate documentation and billing activities within the electronic medical record. Provide education regarding medications, treatments, nutrition, and disease management; address barriers to care; support drug assistance programs; coordinate home health services; distribute and track home health orders; enter home health charges for billing; and promote patient understanding of physician instructions and treatment plans. Obtain outside records and diagnostic results for upcoming visits, review charts to ensure readiness and quality standards, participate in staff meetings and educational sessions, collaborate with team members to provide coverage during absences, and perform additional duties as assigned to support clinic and Department of Medicine operations. Other duties as assigned by clinical nurse supervisor or Director of Nursing Qualifications Minimum Qualifications and Skills: Completion of a program of instruction for licensure as a Practical Nurse required. Completion of BLS Class and re-certify every 2 years. Must be a licensed LPN in Virginia. Previous clinical experience preferred. Montreal Cognitive Assessment (MoCA) certified desirable. Overview Full time position for an advanced medical assistant, providing outpatient care for our patients and assisting our physicians in all clinical areas of the practice. Responsible for front and back office procedures. Also assists Physicians and Nurses with medical examination of patients. This position is located at our Princess Anne Office in Virginia Beach. Responsibilities Train and orient new medical assistants. QA Review of patient charts obtaining all pertinent documentation for return and postpartum patients. Updating patient prenatal lab forms for all follow up patients. Schedule outside patient referrals. Maintain/order supplies for the clinic to include weekly checks for expired materials/medications and/or materials close to expiration. Takes vital signs such as height, weight, temperature, and blood pressure. May draw blood and give injections under the immediate supervision of a Physician or RN. Chaperones when appropriate and assists during certain minor surgeries and diagnostic procedures, such as Pap Smears. Maintains clean, well-equipped examination rooms; cleans and sterilizes instruments. Stocks examination rooms and triage area with medical supplies. Inventories and reorders supplies. Document all information in EMR. Prepare education packets. Check EMR tasks daily. Review patient lab results, discuss abnormal findings with the physician and follow up with patient in regard to treatment as necessary PATIENT CONFIDENTIALITY MUST BE MAINTAINED AT ALL TIMES. OTHER DUTIES AND RESPONSIBILITES Answers incoming telephone calls and refers calls as necessary. Also responsible for taking messages, paging doctors when appropriate, handling hospital consults, and answering inquiries. Schedules appointments with doctors, calls patients to remind and confirm appointments; reschedules patients from the bump list, and schedules residents’ clinics where appropriate. Prepares charts for the next day; creates new patient charts, files and pulls charts and checks charts for referrals and authorization prior to visit. Also prepares charge tickets. Greets patients in a courteous manner, completes clinical intake, and notifies nurses/medical assistants/MDs of arrivals. Checks-in and checks-out patients, obtains and writes referrals, when appropriately trained and directed, and schedules outside appointments with specialists. Gives directions to patients and visitors. Maintains order in reception room. Copies medical records. Provides back-up coverage for medical assistant, billing, etc. Assists RNs and LPNs with administrative duties. Handles pick-up and drop off of medications. Phone triage. Performs other duties as assigned, according to the individual departmental needs. The Licensed Practical Nurse (LPN) is responsible for overseeing and assisting with the day-to-day patient care including but not limited to taking vitals, administering meds/treatments, assisting physicians, doing wound care, patient education, documentation, coordinating care, focusing on comfort and supporting quality initiatives, in a fast-paced office. LPN will also assist with training new staff and a point of contact for medical assistants. Responsibilities Triage incoming patients by obtaining vital signs, following standing triage orders, reconcile medication list, reconcile allergies, make sure immunizations are up to date and prepare patients for the visit. Completes delegated tasks from the current and prior day i.e. patient messages, prescription refills, lab results, referrals, home health requests, medication prior authorizations, patient forms, and physician follow-up. How you make a difference The Nurse Practitioner (NP) under the supervision of the Site Medical Director and other site Physicians, offers a comprehensive range of medical services to incarcerated patients in accordance with established policy and state guidelines. They ensure necessary documentation of services is provided to the Site Medical Director or designee to monitor the provision of clinical services. Additionally, they communicate scheduling changes to the Medical Director and H.S.A. and make arrangements for coverage of medical services if unavailable for an extended period of time. Key Responsibilities Provides medical care to inmates, including conducting exams, chronic care clinics, and infirmary rounds. Maintains accurate and legible progress note documentation in SOAP format and adheres to formulary guidelines. Responds to emergencies and makes logical decisions regarding non-formulary drugs or off-site referrals. Supports existing policies and procedures for admission, transfer, and specialty consultant referrals. Maintains confidentiality and provides input into facility committees as requested. Additional Details -This is a full-time/day shift position. -This position offers an $8500 Bonus for full-time employment. Qualifications & Requirements Education A graduate of an accredited school of nursing An advanced degree in an approved and accredited Nurse Practitioner program. Experience Prior medical/surgical and/or emergency/trauma experience and corrections experience is preferred. Must have the patience and tact to deal with the inmate patient population and ability to work effectively in the corrections environment. Scheduling flexibility is also required to be able to rotate hours and shifts, if needed, and to be called during emergency situations to provide coverage. Licenses/Certifications Must have and maintain current licensure as a Nurse Practitioner within the state of employment. Must have and maintain current CPR certification.
