Overview About the Role The Advanced Practice Nurse (APN) is a skilled health provider who through a written collaborative practice agreement with a physician is credentialed through the Medical Staff Office. As a member of the Medical Staff the APN demonstrates standards of professional practice that includes ethical decision making, job activities related to quality assessment/improvement, self-evaluation, leadership, and research. The APN is self-directed and purposeful in seeking necessary knowledge and skills to develop and maintain competency. The APN in the Surgery Practices utilizes critical judgment in the performance of comprehensive health assessments, differential diagnosis, and the prescribing of pharmacologic and non-pharmacologic treatments in the direct management of patients diagnosed with a variety of surgical conditions/diseases. The APN practice will promote wellness through conducting research, providing education and works autonomously as well as in interdisciplinary teams. Responsibilities What You’ll Do Collects comprehensive patient health data/history including past and present medical, surgical, family, and social history. Diagnostic tests and procedures relevant to the patient’s status are collected, interpreted, and documented at the Electronic Medical Record (EMR). Completes the medication reconciliation process by documenting the current list of medications (including over the counter and nutritional supplements) taken by patient at each visit. Performs and documents an appropriate comprehensive physical assessment of patients which will include a review of systems, evaluation of manifestations and toxicities related to medical conditions and anticoagulation management. In collaboration with the physician, critically analyzes the assessment data/diagnostic tests and determines a diagnosis. In addition, develops a comprehensive plan of care that includes interventions of treatments to attain outcomes. Evaluates patient progress in attaining expected treatment outcomes. Monitors for signs and symptoms related to disease progression/recurrence. Appropriately manages medical and anticoagulation management emergencies according to protocol/policy. Prescribes and/or orders medications. Obtains appropriate consents for surgery and procedures. Ensures completion of all needed pre-operative tests, forms, and documents. Documents assessments, prescribing, plan, modifications to plan, and evaluation of care in the EMR. Qualifications What You’ll Bring Required: Completion of an educational program which is designed and conducted to prepare graduates to practice as an NP/APN with a master’s degree in nursing and eligible for certification by a national certifying body. A current Registered/Professional Nurse license and registration in the State of New Jersey is required. Care is delivered within a family-integrated model, supported by a dedicated pediatric transport team. Notably, this role does not require transport responsibilities. Identifies readmissions within 30 days; performs walking rounds and identifies high‑risk patients; works with Social Work and utilization management to reduce LOS and prevent denials; coordinates and manages distribution of IMM and assists with MOON; conducts or co‑leads care coordination rounds; promotes working relationship with patient/family; acts as advocate; works closely with Social Work and utilization management; serves as a resource regarding patient classification and reimbursement. Demonstrates competency with computer programs; achieves Department Team Dashboard Score Card goals; escalates avoidable days; completes mandatory trainings and in‑services; demonstrates knowledge of safety, security and infection control risks; participates in quality improvement by collecting and analyzing data, identifying trends and suggesting improvements; participates on clinical process improvement teams. Overview Position Overview Legacy Health offers an exciting growth opportunity to join a well-established team of collaborative and energetic Neonatal Nurse Practitioners (NNPs). This role provides the opportunity to practice across both Level III and Level IV NICU settings within the Legacy system. NNPs are integral to delivery attendance and the medical management of high-risk neonates and infants, including daily rounding on NNP service patients. In addition to clinical care, the role includes telemedicine resuscitation coverage for Legacy Level I nursery sites and opportunities to contribute to non-clinical initiatives that support the highest standards of care for NICU patients and their families. Level IV NICU – Randall Children’s Hospital The Level IV, 45-bed NICU is located within Randall Children’s Hospital, a freestanding, nine-story pediatric facility. This unit provides comprehensive care supported by a full range of pediatric subspecialties, including cardiac and neurosurgical services, genetics, and ECMO. Care is delivered within a family-integrated model, supported by a dedicated pediatric transport team. Notably, this role does not require transport responsibilities. Level III NICU – Legacy Salmon Creek Medical Center The Level III, 22-bed NICU at Legacy Salmon Creek Medical Center is a growing unit actively expanding its NNP team. Scheduling is flexible and evolving, with 12-, 16-, and 24-hour shift options available to support work-life balance and team coverage. Scheduling & Practice Model This position is based in the Portland/Vancouver area and requires dual licensure in Oregon and Washington. NNPs rotate between both NICU sites, with frequency tailored to individual preferences while ensuring 24/7 coverage, including nights, weekends, and holidays. A well-established self-scheduling model and structured holiday tracks contribute to provider satisfaction and balance.
