Conduct assessments, implement health care services to be included in each Individual Service Plan (ISP), and respond to changes of condition. Provide training to staff members including universal precautions, infection control, and medication administration. Serve as liaison and advocate with other medical and professional service providers as well as work with external agencies to coordinate services and ensure consistent treatment of individuals receiving care. Provide training for individuals served with topics including self-medication, prescribed treatments, sex education/STD prevention, and smoking cessation. Functions as a coordinator and provider of nursing care in the Maternal Fetal Medicine Department to the obstetric woman and her fetus, who has a known or suspected at-risk pregnancy. In addition, acts as a resource for patients and their families confronting a known or suspected at risk-pregnancy. In collaboration with the perinatologists and the obstetricians, the MFM Nurse Navigator uses assessment, planning and intervention skills to evaluate the patient’s clinical condition, including the use of patient history, diagnostic test results and symptomology during pregnancy. Communicates, as appropriate, with the patients, obstetric and maternal-fetal medicine care providers, obstetric office staff, perinatal sonographers, genetic counselors, and labor and delivery staff, changes in a patient’s clinical condition and testing results. The MFM Nurse Navigator serves as a liaison, educator and patient advocate, and functions within individual patient needs, physician orders, Holy Redeemer Health System policy and procedures, and the Pennsylvania Nurse Practice Act. Provide individualized nursing care to residents according to each resident’s plan of care. Assist in the admission, transfer, and discharge of patients. Administers prescribed medications and treatments as ordered by the physician. Explains procedures and treatments ordered to gain patients’ cooperation and allay apprehension. Observe vital signs to detect deviations from normal and gage progress of the patient. Maintains patients’ medical record on nursing observation and action taken, such as medications and treatments given, reactions, tests, intake, emission of liquids and solids, and vital signs. Records nursing needs of patient on care plan to assure continuity of care. Observes emotional stability of patients, expresses interest in their progress, and prepares them for continuing care after discharge. Perform other duties as required to provide exceptional care to each resident. What You Need Active unencumbered licensure as a Registered Nurse (RN) in Pennsylvania.
