Job Summary The Community-Based Nurse, Lifestyle Medicine & Clinical Research is responsible for promoting health, preventing chronic disease, and improving health outcomes through evidence-based lifestyle interventions, community outreach, and clinical research activities. This unique role combines community health nursing, lifestyle medicine, and research coordination to address population health needs and support health equity initiatives. Working collaboratively with the Program Director and interdisciplinary stakeholders, the Community-Based Nurse conducts community health assessments, develops and implements wellness programs, and coordinates clinical research and quality improvement projects. The ideal candidate is passionate about culturally competent care, community engagement, and empowering individuals and families to achieve sustainable health behavior changes. Qualifications Graduate of an accredited School of Nursing required Bachelor of Science in Nursing (BSN) preferred Current Registered Nurse (RN) license in the State of New Jersey required Current and A Brief Overview The Quality Management (QM) Coordinator's over-aching aim is to advance the organization's commitment to healthcare quality through collaboration and communication.
