Overview Kelemen 10 is a medical-surgical unit that cares for patients who have various needs and disease processes. Both North and South 10 have Telemetry capabilities for our patients. Each side houses 36 patients to a maximum of 72 patients. South 10 has a 12-bed Intermediate Care Unit embedded within the 36-bed unit. INCU has critically care-trained RNs who support the patient's needs and care for ventilated patients. South 10 also has 2 negative pressure isolation rooms with an anti-room that is used when needed for specific isolation needs. We do multidisciplinary rounds Monday – Friday. This involves the patient in their care and allows the team members to collaborate with the patient to give them the best possible care and appropriate discharge to home or other facilities. We have a pharmacist who has an office on the unit to assist the RNs when needed to help avoid delay of care to any patient. Maintain visibility and accountability to increase relationships with contracted inpatient facilities and physician groups. Act as a liaison with AccentCare Hospice Medical Director, Team Physicians and Hospice Physicians on behalf of providers and patients/families. Make inpatient visits at contracted facilities and communicates with Admissions Department, Team Directors and/or Primary Nurse. Provide consultation on hospice and palliative care. As a Registered Nurse Assessment Coordinator, you will play a vital role in ensuring the highest quality of care for our residents. About the RNAC Registered Nurse Assessment Coordinator Role As a Registered Nurse Assessment Coordinator, you will be responsible for coordinating the assessment process for new admissions, as well as conducting comprehensive assessments and updates for residents. You will work closely with the interdisciplinary team to develop and implement individualized care plans, and provide education and guidance to residents, families, and caregivers. RNAC Registered Nurse Assessment Coordinator Responsibilities MDS Coordination: Oversee the timely and accurate completion of Minimum Data Set (MDS) assessments required by federal and state regulations. Care Planning: Partner with interdisciplinary teams (doctors, therapists, and social workers) to build and update individualized care plans for residents.
