Admissions Registered Nurse participates in the information gathering, analysis, education, and form completion aspects of the admissions process 2. Admissions Registered Nurse evaluates patients for hospice appropriateness, e.g. medical records review as requested. 3. Admissions Registered Nurse Obtains agreement for admission of referred patients and families who meet admission criteria. 4. Admissions Registered Nurse analyzes data collected by the admission staff or ANCHOR HEALTH clinicians to facilitate/obtain admission orders 5. Admissions Registered Nurse builds effective referral source relationships to support the high quality and responsiveness expectations associated with ANCHOR HEALTH. 6. Admissions Registered Nurse maintains clinical and professional competence. 7. Admissions Registered Nurse takes the initiative in developing effective relationships internally with the appropriate admissions and patient care resource persons. 8. Admissions Registered Nurse utilizes interpersonal skills to help appropriately educate and address any questions the patient and/or patients’ family has concerning hospice and palliative care. 9. Qualifications • Master's or doctoral degree in nursing • Current NP License within the State Responsibilities • The Nurse Practitioner (NP) is an experienced licensed professional who works in collaboration with the Hospice Medical Director/Physician to deliver hospice and/ or palliative care to patients and families • Performs prescribed medical treatments, including patient examination, diagnoses, medication prescription, pain management and symptom control; conducts assessments and evaluations, and provides education and supportive care to hospice teams, patients and caregivers • Performs comprehensive patient examinations, evaluations, assessments and diagnosis • Assesses for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action to control and manage patient symptoms and needs • Makes adjustments to the already established plan of care and appropriate services, provides appropriate follow- up to all patients • Demonstrates flexibility in scheduling, and readily accepts new and emergency assignments as requested • Performs timely face to face assessments with patients • Demonstrates positive interpersonal relation in communicating with physicians and co-workers to promote quality patient care • Completes all documentation in an accurate, legible and timely manner • Provides quality patient care consistent with standards of nursing practices • Complies with hospice policies and procedures, laws and regulations and standards of practice

Registered Nurse (RN)- Outpatient Hospice, Full-Time, 8:30 AM-5:00PM

$51.50 - $82.64 / hour
Provides direct nursing care including assessment, plan of care, education, and evaluation for patients certified as having a terminal illness who have elected the Hospice benefit. Performs patient duties necessary to maintain the flow of patient care as well as coordinates the delivery of care provided by other care team members and disciplines. The Hospice Registered Nurse also provides supportive care to patient’s families and/or significant others.
