Observe ethical and legal guidelines for safeguarding confidentiality of patients. Assess patient needs and physical status each skilled visit through health data assessment and patient interview. Re-evaluate patient needs through physical assessment, response to therapy and any updated physician’s orders. Inform physician of any changes in patient condition, response to care, progress etc. Obtain and clarify orders for Plan of Treatment revisions. Prepares clinical notes and updates the primary physician when necessary and at least every sixty days. Completes And submits all documentation in accordance with policy and procedures. Communicates with the physician regarding the patient’s needs and reports and changes in the patient’s condition; obtains/receives physician’s orders as required. Communicates with other health related persons to coordinate care plan. Participates in on-call duties as defined by the on-call policy. Ensures that arrangements for equipment and other necessary supplies and services are available. Instructs, supervises and evaluates home health aide/personal care aide care provided every 14 days or as stipulated by law and regulation. Instructs, supervises and evaluated LPN/LVN care provided at least every 30-days. Exercises professional management. And coordinates care between and among involved parties in home, community providers and pediatric tertiary specialists. QUALIFICATIONS: Bachelor's degree of Nursing from an accredited college/university Current active and unrestricted nursing license Pediatric care experience in the acute and/or home care environment Valid driver’s license Current certification in Basic Life Support for Health Care Professionals. Minimum of 1 year of recent acute or home care experience, preferably in an institutional setting. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills per competency checklist. Job Description Short Description The Inpatient Care Manager is responsible for coordinating patient care across the continuum of illness to ensure high-quality outcomes and efficient use of healthcare resources. Working closely with physicians, nurses, and interdisciplinary teams, this role leads care planning, utilization review, and discharge coordination while proactively addressing barriers to care. By combining clinical expertise with collaboration and oversight, the Inpatient Care Manager helps ensure safe, timely, and patient-centered care. Description Responsible for providing care and services of selected member populations across the continuum of illness. Ensures effective utilization and monitors health care resources. Manages the interdisciplinary team to achieve optimal clinical and resource outcomes. Works with the Supervisor/Manager of Case Management to assess, plan, implement, coordinate, monitor and evaluate services and outcomes to maximize the health of the patient. Principal Accountabilities Manage, coordinate, and facilitate patient care through comprehensive patient and family assessments and multidisciplinary collaboration. Perform daily chart reviews, utilization reviews, and coordinate discharge planning activities, ensuring thorough and accurate clinical documentation in the electronic medical record. Lead and participate in interdisciplinary patient care rounds, identifying and resolving quality of care issues, barriers to care progression, and discharge delays. Formulate, coordinate, and implement individualized plans of care and discharge plans to ensure safe, timely, and effective transitions. Collaborate with the interdisciplinary healthcare team to manage and support patient care plans. Perform variance tracking and outcome analysis to assess the effectiveness of clinical pathways and identify opportunities for improvement. Serve as a representative of the department and participate in hospital and community outreach activities as appropriate. Perform other related duties as assigned.
