Completes, maintains and submits accurate and relevant clinical documentation regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate. Communicates and collaborates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. Works closely with members of the IDG to provide continuity of care and support for the patient and family. Collaborates with vendors to obtain DME, supplies and medications when needed after-hours. Collaborates with the hospital and other community residential staffing to adjust interventions and plans of care as needed. Conducts after hours hospice admissions as necessary. Participates in mandatory meetings as required. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-services. Shifts: 6AM-9AM 4PM-7PM Overview The Certified Medication Technician (CMT) assists residents with medications as allowed under State guidelines and is responsible for safe administration of prescribed medications and treatments. The CMT is responsible for the safe administration of prescribed medications and treatments as listed on Medication Administration Record (MAR). The CMT meticulously correct records of care including Medication Observation Record. The CMT provides personal care and personal care to residents as needed/assigned and ensures delivery of care and services to residents in the community to include Activities of Daily Living (ADLs), room cleanliness, scheduled activities and dining services and individual service plan need of each assigned resident. Qualifications: The CMT must be in good standing and meet all applicable federal and state certification requirements. Completion of high school or GED equivalent. Must be able to read, write and follow oral and written communication Must speak and understand English Positive attitude toward those diagnosed with mental illness as well as the elderly. Inspect medications weekly to ensure expired medications are removed. Verify current MARs are compared to new MARs at change over time and notify RCD of discrepancies. Remove discontinued medication from medication cart and give to RCD. Provide personal care to residents when needed, including, but not limited to, bathing, skin care, oral hygiene, grooming, dressing, incontinence care, eating, etc. Provide general ADL care including, but not limited to vital signs, temperature, blood pressure, and weight. Document appropriate information in daily log as required, notify Shift Supervisor (SS) and RCD of concerns. Answer calls and assist residents with needs as assigned. Job Summary The Surgical Technician – Certified works collaboratively with the surgical team to facilitate safe, effective, and high-quality patient care in the operating room. This role prepares and maintains the sterile field, anticipates surgical team needs, and ensures proper handling of surgical instruments and supplies. The Surgical Technician – Certified also supports OR efficiency through equipment readiness, inventory management, and adherence to infection prevention protocols. Essential Functions Prepares the operating room by ensuring surgical instruments, equipment, and supplies are properly set up and available based on the procedure and physician preference card. Maintains a sterile field throughout the surgical procedure, applying principles of Universal Precautions, asepsis, and sterile technique. Anticipates the needs of the surgeon, passing instruments and supplies accurately and efficiently during surgical procedures. Handles surgical instruments and equipment according to manufacturer instructions and hospital policy, including inspection, point-of-use cleaning, and sterilization. Performs and documents sponge, needle, and instrument counts in collaboration with the circulating nurse to support patient safety. Assists with patient positioning, draping, and transfer under the direction of the RN to ensure patient safety and procedural readiness. Supports ongoing maintenance and accuracy of physician preference cards and procedural documentation. Restocks supplies, cleans and returns equipment to designated storage areas, and prepares rooms for subsequent cases to ensure OR efficiency. Performs other duties as assigned. Qualifications Technical School graduate of Surgical Technology or Practical Nursing program required or Associate Degree in Surgical Technology or Applied Sciences required 0-2 years of operating room surgical technician experience required Knowledge, Skills and Abilities Expertise in surgical techniques, instrumentation, and sterile processing. Ability to anticipate the needs of the surgical team and respond quickly during procedures. Strong organizational and time management skills to maintain efficient workflow.
