As a hospice Registered Nurse Case manager, you will perform ongoing patient assessments, maintaining thorough and diligent notes and records regarding the effectiveness of each patient's care plan. You will also keep in constant contact with patient family members and alternative caregivers. Additional responsibilities for the hospice RN Case Manager will include: Providing nursing care with an emphasis on pain and other management. Supervising Home Health Aides. JOB SUMMARY The certified nursing assistant is an integral member of the health care team who delivers quality care through collaboration and communication with team members.
