Maintains a computerized, evolving plan of care based on nursing assessment, the medical plan of care and patient need. The plan is initiated according to department standards. • c. Provides patient care, which demonstrates a comprehensive knowledge of normal physiology and disease pathophysiology of the pregnant and postpartum woman and the newborn. Appropriate nursing and medical interventions and knowledge of the principles of growth and development over the lifespan. • d. Implements patient plan of care based on evidence-based practice. • e. Documents patient response to treatment and effectiveness of care. • f. Provides teaching and discharge planning based on assessment of self-care knowledge/ability on an ongoing basis. • g. Ensures appropriate completion of chart and other documentation including transcription of physician orders and computer entry. • h. Ensures that unapproved abbreviations are not used when documenting on patient’s medical record. • i. Demonstrates appropriate skill in utilization of the computer for automated order entry, data retrieval, and medication administration, nursing care planning and intervention and interdepartmental communication as determined by defined access levels. • j. Communicates and collaborates with patients, physicians, family members, and other health care disciplines in order to achieve excellence in patient care during hospitalization. • a. Utilizes effective interpersonal skills including interaction with staff through understanding, talking, and hearing, seeing and listening. • b. Involves patient/significant other in all aspects of the plan of care, serving as advocate when necessary. • c. Consistently and continuously communicates, consults and shares pertinent information regarding changes in patient status and patient care. • d. Maintains equipment in good working order, reports equipment failure, and initiates corrective action. Safely administers medications and treatments as ordered according to hospital policy and formulary using the appropriate principles of safe medication administration, 2 patient identifiers. • e. Safely handles various types of drugs and body fluids using knowledge of appropriate standard precautions. • 4. PURPOSE AND SCOPE: The professional registered nurse Inpatient RN CAP 2 may be an entry level designation into the Clinical Advancement Program (CAP) for new employees who meet the RN CAP 2 criteria or attained through advancement from CAP 1. As a hospice Registered Nurse Case manager, you will perform ongoing patient assessments, maintaining thorough and diligent notes and records regarding the effectiveness of each patient's care plan. You will also keep in constant contact with patient family members and alternative caregivers. Additional responsibilities for the hospice RN Case Manager will include: Providing nursing care with an emphasis on pain and other management. Supervising Home Health Aides. Referring patients' families to applicable community resources. Maintaining strict patient confidentiality. Providing patient care directives to family members and alternative caregivers. Requirements – As a hospice Registered Nurse, you must have absolute integrity and sound judgment. You will act as a patient advocate, working independently to ensure your patients are physically comfortable and content.
