JOB DESCRIPTION SUMMARY: The Certified Nurse Aide (CNA) is a member of the Nurse Next Door team and works under the supervision of the Care Designer or a nurse and performs personal care services contained in a care plan written by a licensed health professional, employed or contracted by the agency. The CNA is responsible for observing clients, reporting these observations and documenting observations and care performed and will be assigned in a manner that promotes quality, continuity and safety of a client’s care. JOB FUNCTIONS/RESPONSIBILITIES: Responsibilities of CNA (not all encompassing) Bathing, grooming, including oral care Assist the client with dressing and undressing Peri-care, including the use of the bedpan and urinal Provide catheter care including changing the urinary catheter bag Empty ostomy bags, or changing bags that do not adhere to the skin Assist in feeding the client, if necessary Take and record vital signs (temperature, pulse, respirations and blood pressure) Support good skin care to prevent skin breakdown (i.e. turn and reposition client, actively encourage movement) Assist clients with active and passive range of motions as defined by Care Plan Provide clients with help walking and transferring; assist with the use of devices for aid to daily living such as a wheelchair or walker Observe client’s physical, mental and emotional conditions; report any change to the Care Designer Assist client in the self-administration of medication Perform light housekeeping; maintain a clean, safe and healthy environment, to include light cleaning and straightening of the bathroom, straightening the sleeping and living areas, washing the client's dishes or laundry, change bed linens and making up the client’s bed Perform meal preparation and clean up Follow principles of infection control and Universal Precautions Provide transport to doctor appointments, outside activities and shopping CLIENTS RIGHTS Ensure client’s rights are adhered to.
